The cumulative burden of oral corticosteroid side effects and the economic implications of steroid use

Abstract: Oral corticosteroids (OCS) are a key part of therapy regimens for a diverse variety of conditions. Despite their efficacy, they are associated with a wide variety of adverse events. The purpose of this review was to identify the range of adverse events that have been reported to be related to oral corticosteroids, examine the factors that influence their incidence and estimate the economic burden caused by these adverse events. In 61 identified studies, 21 different categories of OCS related adverse events wer… Show more

“…20 The use of OCS therapy chronically is not only associated with a significant increase in adverse events but is also associated with an increased economic burden. 21 In addition, there are individuals who elect not to use maintenance controller therapy but would rather use frequent courses of OCS to treat exacerbations. This group requires education with regard to the efficacy of maintenance controller therapy in reducing the risk of asthma exacerbations and improving asthma control.…”

“…Steroids and cyclooxygenase inhibitors have long been used as major therapeutical antiinflammatory agents. However, they often cause serious side effects in patients (4,5). Thus, developing unique antiinflammatory agents is currently in high demand.…”

“…I nflammation is a hallmark of many human diseases, including atherosclerosis (1), pulmonary inflammatory diseases (e.g., chronic obstructive pulmonary disease, asthma) (2), arthritis (3), infectious diseases, and cancer (4,5). Steroids and cyclooxygenase inhibitors have long been used as the main therapeutical antiinflammatory agents, but they are frequently associated with significant detrimental effects in patients (4,5).…”

“…The side effects seen with corticosteroid therapy include obesity, diabetes, skin atrophy, disturbed wound healing, cataracts, glaucoma, and bone loss [338]. The risk of osteoporosis is dose-related and can be reduced by vitamin D and calcium supplementation [339].…”
