Introduction. Although several papers reported a wide range of negative outcomes among patients with both schizophrenia and SUD, only a few studies evaluated the impact of SUD on psychopathology and thus on the length of first-hospitalization. Objectives.To compare clinical expression of first-episode of schizophrenia between inpatients with and without SUD, giving close attention to the length of stay.Methods. 130 inpatients at first-episode of schizophrenia were assigned to SUD or not SUD group depending on SUD diagnosis and were assessed through BPRS at admission, during hospitalization and at discharge. Cross-sectional and longitudinal statistical analysis were performed to investigate differences between groups and also a linear regression was used to evaluate relationship between length of stay and BPRS scores.Results. During the hospitalization there was a more marked improvement of BPRS total score in patients without SUD with a lower length of hospitalization (P<0.001). BPRS score during hospitalization was predictor of higher length of stay.Discussion. The higher severity of psychopathology during the hospitalization suggest possible overlaps of substance induced and withdrawal symptoms. Conversely, higher levels of conceptual disorganization during the whole period of hospitalization also support a more severe and pharmacho-resistant expression of schizophrenia among SUD patients.

SummaryWe describe the work of a psychodynamic psychotherapy group for individuals affected by chronic schizophrenia in our community-based rehabilitation and recovery service. Despite presenting with negative symptoms, over time patients made good use of the psychodynamic setting: they attended, expressed emotions, made connections and reflected within the group on their personal experience of mental ill health. We discuss the rationale and indications for psychodynamic psychotherapy for individuals with severe and enduring mental illnesses. We also reflect on the challenges we faced working psychodynamically with psychosis and on the necessary adaptation of the approach to work with chronic psychosis. We present vignettes from group sessions to illustrate themes that we identified in the therapy, making links with the literature. We conclude that the unstructured setting of the psychodynamic group approach can help individuals affected by most severe forms of psychosis to make sense of their experiences and use the space.Learning Objectives•Understand the rationale for psychodynamic group interventions for psychosis.•Understand what adaptations of psychodynamic approaches are necessary to work with patients with psychosis.•Change attitudes toward offering psychodynamic interventions for psychosis.

This study examines self-image in a sample of first-episode psychosis (N = 97) at baseline and as predictor for outcome. Self-image was measured using the Structural Analysis of Social Behavior Questionnaire (=SASB-Intrex) at the perceived best and at worst state. The main findings are that persons with psychosis and a more positive self-image at their perceived best state have a better remission from negative and positive psychotic symptoms and an increased level of social functioning after one year in treatment. Further, some support was found for the presence of defensive processes in forming the self-image in psychosis, as reduced acknowledgement of present illness and grandiosity were associated with a less negative self-image at worst state. This defensive strategy might be helpful in remission, as a less negative self-image at worst state was related to better remission in positive psychotic symptoms after one year in treatment.
