Audit is a core component of the clinical governance programme, the mechanism to improve the quality of the service and the performance of clinicians in the NHS. The Royal College of Physicians (RCP), UK, has produced national guidelines setting standards for botulinum toxin therapy in adult spasticity. Such therapy is an essential element in treating spasticity. This article critically explores the experience of the first 3 years of a new service at one acute trust. This is the first reported audit on spasticity or botulinum toxin therapy. The results highlighted areas for improvement and consequent specific action plans have been drawn up for implementation. The data, together with other quality indicators, will support the team to engage with the service commissioners for improvement and further investments in this adult spasticity service. Future audit data from other services can potentially contribute to benchmarking of spasticity services. The article also considers and explores the potential role of physiotherapist or nurse injectors of botulinum toxin therapy for spasticity treatment; and future research ideas are generated.
