The high incidence, mortality, and disability associated with ischemic stroke pose a significant threat to human health. The intestinal microbiota significantly influences the onset, progression, and prognosis of ischemic stroke. Gut flora plays a pivotal role in brain–gut interactions. The reflection of changes in the gut and brain caused by gut microbes facilitates the investigation of early warning biomarkers and potential therapeutic targets for ischemic stroke. In this narrative review of the relationship between gut microbiota and ischemic stroke, we primarily discuss three topics, grounded in real‐world human and animal studies. First, we examined the relationship between ischemic stroke and intestinal microbiota and its metabolites, delineate the overall characteristics of intestinal microbiota dysregulation in ischemic stroke, and assess the potential clinical value, prevailing research controversies, and unique phenomena of intestinal microbiota metabolites such as trimethylamine N‐oxide and short‐chain fatty acids in ischemic stroke. Second, we explored the potential communication pathways between intestinal flora and ischemic stroke based on the brain–gut axis, encompassing metabolic pathways, immune pathways, and neural pathways. Finally, we encapsulated the factors influencing the severity of ischemic stroke via intestinal flora, the pharmacological and nonpharmacological interventions that modulate intestinal flora in treating ischemic stroke, and the current research landscape of intestinal flora in the context of ischemic stroke sequelae.

BackgroundPrevious studies have shown that gut microbiota dysbiosis could affect clinical prognosis through an unknown mechanism. However, the causal relationship between the gut microbiota and the functional outcome after ischemic stroke remains unclear. We aimed to investigate the causal association between the gut microbiota and the functional outcome after ischemic stroke using Mendelian randomization (MR).MethodsGenetic instrumental variables associated with 211 bacterial traits were obtained from the MiBioGen consortium (N = 18,340). Data from genome-wide association studies (GWAS) for functional outcome after ischemic stroke were obtained from two phenotypes (i.e., overall stroke outcome and motor recovery). The inverse variance weighted method was used to estimate the causal association. Enrichment analysis was conducted based on the results of the MR analyses.ResultsThe genetically predicted family Peptostreptococcaceae (OR = 0.63, 95% CI = 0.41–0.98, p = 0.038) and the genera LachnospiraceaeNK4A136 group (OR = 0.65, 95% CI = 0.43–1.00, p = 0.048), LachnospiraceaeUCG004 (OR = 0.54, 95% CI = 0.33–0.90, p = 0.017), and Odoribacter (OR = 0.40, 95% CI = 0.21–0.77, p = 0.006) presented a suggestive association with favorable functional outcome, while the genera Eubacterium oxidoreducens group (OR = 1.77, 95% CI = 1.11–2.84, p = 0.018) and RuminococcaceaeUCG005 (OR = 1.85, 95% CI = 1.15–2.96, p = 0.010) were associated with unfavorable functional outcome. The genetically predicted family Oxalobacteraceae (OR = 2.12, 95% CI = 1.10–4.11, p = 0.025) and the genus RuminococcaceaeUCG014 (OR = 4.17, 95% CI = 1.29–13.52, p = 0.017) showed a suggestive association with motor recovery, while the order Enterobacteriales (OR = 0.14, 95% CI = 0.02–0.87, p = 0.035) and the family Enterobacteriaceae (OR = 0.14, 95% CI = 0.02–0.87, p = 0.035) were associated with motor weakness. Enrichment analysis revealed that regulation of the synapse structure or activity may be involved in the effect of the gut microbiota on the functional outcome after ischemic stroke.ConclusionsThis study provides genetic support that the gut microbiota, especially those associated with short-chain fatty acids, could affect stroke prognosis by mediating synapse function. Our findings suggest that modifying the composition of the gut microbiota could improve the prognosis of ischemic stroke.

ObjectiveThis study aims to clearly define the effects of Banxia-Yiyiren on the gut microbiota and its metabolites in a para-chlorophenylalanine-induced insomnia model and the possible underlying mechanisms involved.Materials and methodsWe employed 16S ribosomal ribonucleic acid (rRNA) gene sequencing combined with metabonomic analysis to explore the mutual effects of the PCPA-induced insomnia model and the gut microbiota and the intrinsic regulatory mechanism of Banxia-Yiyiren on the gut microbiota and metabolites in the PCPA-induced insomnia model.ResultsBanxia-Yiyiren was identified by mass spectrometry to include amino acids, small peptides, nucleotides, organic acids, flavonoids, fatty acids, lipids, and other main compound components. The elevated plus maze (EPM) test results revealed that high-dose Banxia-Yiyiren may increase willingness to explore by improving anxiety-like symptoms caused by insomnia. Through 16S rRNA gene sequencing, at the phylum level, compared with those in G1, the relative abundances of Bacteroidota and Proteobacteria in G2 increased, whereas the relative abundance of Firmicutes decreased. At the genus level, compared with those in G1, the relative abundances of Prevotella_9, Prevotella, Ralstonia, Escherichia-Shigella, and UCG-005 in G2 increased, whereas the relative abundances of Lactobacillus, Ligilactobacillus, Alloprevotella, Blautia, and Prevotellaceae_NK3B31_group decreased. The metabolomics analysis results revealed 1,574 metabolites, 36.48% of which were classified as lipids and lipid-like molecules, 20.76% as organic acids and their derivatives, and 13.36% as organic heterocyclic compounds. The correlation between the top 20 differentially abundant metabolites in the G1–G2 groups was greater than that between the G3–G2 and G6–G2 groups. Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis revealed that the main differentially abundant metabolites in each group were significantly enriched in various pathways, such as amino acid metabolism, adenosine triphosphate (ATP)-binding cassette (ABC) transporters, protein digestion, and absorption. Additionally, there was a significant Pearson correlation between the genus-level differences in the gut microbiota and the differentially abundant metabolites among the G1–G2, G3–G2, and G6–G2 groups.ConclusionThis study preliminarily verified that the PCPA-induced insomnia model is closely related to gut microbial metabolism and microecological disorders, and for the first time, we confirmed that Banxia-Yiyiren can act on the gut microbiota of PCPA-induced insomnia model rats and alleviate insomnia and anxiety by regulating the species, structure, abundance, and metabolites of the gut microbiota.
