The Dimensionality of the 12-Item General Health Questionnaire (GHQ-12): Comparisons of Factor Structures and Invariance Across Samples and Time

Abstract: Because of its brevity, the 12-item General Health Questionnaire (GHQ-12) has become one of the most popular and used measure for detecting psychological distress. Originally intended as a unidimensional measure, the majority of subsequent factoranalytic studies have failed to support GHQ-12 as a unitary construct and have instead proposed a plethora of multidimensional structures. In this study, we further examined the factor structure in two different military samples, one consisting of crewmembers from four… Show more

“…General health questionnaire 12 (GHQ-12) is widely used as a self-report measure of mental health (Goldberg et al, 1997(Goldberg et al, , 2013Hankins, 2008). A large number of studies in the general adult-, clinical-, work and student populations have provided support for its validity across samples and contexts (Aalto et al, 2012;Adlaf et al, 2001;Donath, 2001;Firth, 1986;Goodwin et al, 2013;Gorter et al, 2008;Malt, 1989;Nerdrum et al, 2006) and translated from English to several other language, among these Norwegian (Hystad & Johnsen, 2020;Malt et al, 1989). Six items of the GHQ-12 are phrased positively (e.g.…”

“…We used the standard scoring method (0-0-1-1), which gives a possible score ranging from 0 to 12. We used the total score, as recommended by Hystad and Johnsen [19], a higher score indicates a greater degree of psychological distress. A threshold of 3 or more has been used to identify the presence of distress in other studies [20][21][22].…”

“…Twelve item version of General Health Questionnaire (GHQ-12) survey was used as described previously (21). The GHQ-12 is a well-established method to quantify stress and has been used in a wide range of conditions (22,23). Our questionnaire was designed to assess four major factors based on the relevant factors during the early stage of pandemic.…”
