The distal radius palmar cortical angle in adolescent versus adult populations

Abstract: Purpose: Volar distal radius plates are pre-contoured to aid restoration of anatomy during open reduction internal fixation of distal radius fractures. Incorrectly contoured plates can result in malreduction, leading to malunion, pain, and loss of function. The purpose of this study was to investigate whether adolescent distal radius anatomy on the palmar cortical surface differs from that in adults, in order to determine whether adult plates are suitable for use in adolescents. Methods: Lateral wrist radiogra… Show more

“…In unilateral fractures an x-ray of the contralateral wrist is obtained for surgical planning. Dorsal tilt, radial inclination, ulnar variance, and translocation (coronal/frontal) and the palmar cortical angle 15 are measured from both wrists to determine the desired correction.…”

“…Anatomic volar plates for distal radius fractures are unsuitable for this procedure since the plate is positioned proximal to the physis and because the palmar cortical angle of an adolescent differs from that of an adult. 15 For this procedure I currently use Evos-Mini 2.4 to 2.7 mm T-plate (Smith-Nephew Inc., Andover, MA). Two cortical screws are used to temporarily attach the plate to the distal fragment.…”
