Hair and scalp disorders represent a considerable complaint among patients attending dermatologic clinics. 1 Human scalp hair is subjected to a variety of cosmetic treatments and provides a diagnostic clue to variable diseases. 2 A significant variation in hair parameters was reported among Europeans, Asians, East Asians, and Africans regarding cross-sectional shape, cuticle dimensions, and melanosomes' distribution, which could be attributed to genetic diversity. 3 Trichoscopy (dermoscopy of the hair and scalp) is a noninvasive, cost-efficient technique that visualizes subtle details and allows identification of hair and scalp abnormalities while reducing the need for hair sampling or scalp biopsies. 4,5 It can be performed with a manual dermoscope (×10 magnification) or a videodermoscope (up to ×1000 magnification). 6 Several studies have evaluated the potential value of trichoscopy in the diagnosis of hair and scalp disorders, on top of which, androgenic alopecia, 7 alopecia areata, 8 and inherited structural hair shaft abnormalities. 9 Proper knowledge of normal trichoscopic hair parameters is essential for an adequate diagnosis of these disorders. 4 However, there is no clear consensus on normal hair trichoscopic features, since these norms differ according to age, gender, and ethnicities. 3

Female Pattern Hair Loss in Female and Male: A Quantitative Trichoscopic Analysis in Chinese Han Patients

Objectives: To investigate the trichoscopic features of female pattern hair loss (FPHL) in Chinese Han patients and analyze the difference between male and female patients with FPHL.Materials and Methods: Trichoscopic images were taken in four different scalp areas, including right frontal hairline, vertex, right parietal and occipital areas. Hair density, hair shaft diameter, vellus hair ratio and single hair follicle unit ratio were counted manually and analyzed.Results: Seventy-three subjects were enrolled in this study, including 38 patients with FPHL (28 females and 10 males) and 35 normal controls without hair loss. The hair density and hair shaft diameter of FPHL patients reduced in the whole scalp. Vellus hair ratio and single hair follicle unit ratio were both increased in FPHL compared to normal controls. The vertex was the most affected area and the hair shaft diameter showed the most significant difference. Parietal and occipital area were also affected in FPHL. The reduction or increase was correlated with the severity of Ludwig staging. Very few gender differences were detected in male and female FPHL patients.Conclusion: FPHL patients showed decreased hair density and hair shaft diameter, accompanied by increased vellus hair ratio and single hair follicle unit ratio. Parietal and occipital area can be also affected in FPHL, though not as severe as in vertex area. FPHL in male basically has the same characteristic as those in female patients.Limitation: The main limitation of the study is the small sample size which only enrolled 10 male FPHL patients, in comparison to the female cases. The findings could not be representative of the normal population with the limited sample size.

Background: Trichoscopy is a simple, noninvasive tool to help in the diagnosis of various hair and scalp disorders. There is paucity of data on the normal trichoscopic parameters of hair density and diameter in the Indian population. Aim: The aim of this study was to establish the trichoscopic patterns of hair and scalp in healthy Indian males and to provide a framework for future reference. Materials and Methods: Quantitative trichoscopic analysis using a videodermoscope was done on healthy Indian males with no hair/scalp complaints. Images were taken from four areas: frontal, occipital, and both temporal areas, under 20x and 70x magnification. The following parameters were analyzed by the software: hair density and thickness; percentage of thin, medium, and thick hairs; percentage of anagen and telogen hairs; vellus hair density and hair count; follicular unit density; percentage of follicular hair units; single, double, triple, and four hairs per follicular unit; and distribution and pattern of scalp vasculature. Results: A total of 120 healthy males were recruited in the study with a mean age of 24.6 years. The hair density was the highest in the frontal and occipital areas, while the hair thickness was similar in all the sites evaluated. The follicular unit density was the highest in the frontal and occipital areas with the temporal area having the highest number of single hair follicular units. The scalp vasculature varied with the area examined, the most common pattern being the pin-point type. Limitations: It was a hospital-based study with a relatively small sample size. The software used in the study cannot detect gray hairs. Conclusion: Our study has attempted to fill the gaps in the medical literature by proposing comparable reference values for trichoscopy in healthy Indian males.
