This paper gives an update on the local distributions of HCV genotypes in Aydin province of Turkey, provides a comparison with the previous records, and discusses the potential causal reasons shaping the evolving genotype profiles. Patient files from 2011 to 2016 were retrospectively analyzed, and newly detected cases were documented. Out of 286 patients, male and female ratios were determined to remain nearly the same (~50%). Genotype 1 was still the most common (90.2%), followed by genotype 3 (5.9%), genotype 2 (2.1%), and genotype 4 (1.4%) in frequency. There were international patients (4.50%). One patient had genotyped 2+3 together. Genotypes 4 and 2+3 were detected for the first time, and the patients with genotype 4 were interestingly all male and also domestic individuals. However, these patients traveled or lived abroad in the past due to occupational reasons, thereby likely acquired the infection while abroad. HCV surveillance system is currently inadequate and some infected patients may go undetected in the province. Remapping the regional distribution of HCV genotypes from time-to-time is required for identifying the local dynamics and causes leading to it. This process enhances the clinical preparation and readiness for the better management of the disease.

Introduction: A higher percentage of sustained virologic response (SVR) has been reported with the introduction of direct-acting antivirals (DAAs) to the treatment of hepatitis C in recent years. However, there are still relatively limited data on the effectiveness and safety of the use of DAAs in hemodialysis patients. The aim of this study was to evaluate the efficacy and safety of chronic hepatitis C treatment with paritaprevir/ritonavir/ ombitasvir and dasabuvir (3D) in hemodialysis patients. Materials and Methods: Hemodialysis patients who were treated with 3D regimen between July 2016 and October 2018 were evaluated retrospectively. Patients coinfected with hepatitis B or human immunodeficiency virus and those with cirrhosis were excluded. Serum hepatitis C virus (HCV) RNA and alanine aminotransferase levels of the patients were recorded after one and three months of therapy. SVR was defined as negative HCV RNA at three months after cessation of HCV treatment. Any drug-related alterations in clinical or laboratory findings during the treatment period were evaluated as side effects. Results: Fifteen patients who were treated with the 3D regimen during the study period were included. Genotype 1b and 1a were detected in 12 and three patients, respectively. Ribavirin was added to treatment for genotype 1a-infected patients. Hemoglobin levels were reduced in two of the three patients who received ribavirin. One patient had macular rash and another patient complained of fatigue. No serious side effects were observed. At the end of treatment, a low level of HCV RNA (63 IU/ml) was detected in only one patient. At the end of treatment, HCV RNA negativity was achieved in 12 out of 13 patients whose HCV RNA quantitation data were available. End-of-treatment success rate was 92%. SVR was achieved in all of the patients at three-months after treatment cessation (100% SVR12). Conclusion: This study shows that the 3D regimen is safe and effective in the treatment of hemodialysis patients infected with hepatitis C.

Objectives: In this study, we evaulated the distribution and alteration of hepatitis C virus (HCV) genotypes throughout years which has a clinical importance in the treatment and follow-up. Materials and Methods: Test results obtained from blood samples sent to the molecular microbiology laboratory at Aydın State Hospital for HCV genotype determination were analyzed retrospectively. A total of 182 samples collected between 2014 and 2018 were enrolled in the study. The determination of genotype and viral load of the samples were performed by real time polymerase chain reaction. Results: 53.8% (98/182) of the samples were collected from male patients and 46.2% (84/182) from female patients. The mean age of the patients was 58.5±15.5 years. 69.2% of the samples were genotype 1b, 18.1%-genotype 1a, 2.2%-genotype 1 (those different from subtype 1a and 1b), 1.7%-genotype 2, 7.2%genotype 3, and 1.7% of the samples were genotype 4. Conclusion: In the present study, genotype 1 was the most common genotype (89.5%). Additionally, we have observed a decrease in the frequency of genotype 1b and a slightly increase in the frequency of other genotypes. Determination of HCV genotypes is important for treatment and prognosis of HCV infections.
