The DSM Diagnostic Criteria for Pedophilia

Abstract: This paper contains the author's report on pedophilia, submitted on June 2, 2008, to the work group charged with revising the diagnoses concerning sexual and gender identity disorders for the fifth edition of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders (DSM). “…For my purposes, the sexual attraction part of the definition is the key consideration in operationalizing pedophilia. There has been recent debate about the existence of hebephilia, a sexual age orientation for pubescent children, that is, children who are beginning to show signs of physical maturity such as breast budding in girls or changes in the scrotum and penis for boys (Blanchard, 2010;Franklin, 2009;Seto, 2010). There is no word coined yet for individuals who are attracted to both mature and immature persons.…”

Comparing pedophilia to other paraphilias, the acting out of the preference can be seen as an idiosyncratic clinically relevant criterion (Blanchard 2010).…”

“…The Paraphilias Subworkgroup of the Sexual and Gender Identity Disorders Work Group for DSM-5 has proposed replacing the current diagnostic construct, Pedophilia, with Pedohebephilic Disorder, both to reflect the existence of pedohebephiles and hebephiles and also to bring the DSM into closer agreement with the ICD, which currently defines Paedophilia as ''A sexual preference for children, boys or girls or both, usually of prepubertal or early pubertal age''(ICD-10 F65.4; emphasis added). My original report to the Paraphilias Subworkgroup suggesting the replacement of Pedophilia with Pedohebephilic Disorder was published as Blanchard (2010a). The rationales for all notable changes to the diagnostic criteria for all the paraphilias may be found on the DSM-5 Website maintained by the American Psychiatric Association, http://www.dsm5.org.…”
