The Dynamic Nature of Mobility Disability in Older Persons

Abstract: Mobility disability in older persons is a highly dynamic process, characterized by frequent transitions between states of independence and disability. Programs designed to enhance independent mobility should focus not only on the prevention of mobility disability but also on the restoration and maintenance of independent mobility in older persons who become disabled.

“…Mobility disability is a dynamic condition and transitions between different levels of ability to walk 1/4 mile are common among older adults. 17 Interventions can improve objective ability to walk 1/4 mile; further studies are needed to determine if improvements in self-reported walking ability are associated with corresponding decreases in mortality and health care utilization. In addition, future studies should compare the performance of walking difficulty with other methods of screening risk of disability, death, and future utilization.…”

“…Some researchers have focused on endurance, suggesting that individuals who are unable to walk 400 m at once have a greater level of disability than those individuals who can walk for this extended distance [30]. Others have used both, suggesting that speed on a 4 m walk test can predict success on a 400 m walk test for older adults [31].…”

“…These periods of transition or functional mobility pose challenges to posture and balance that could result in a fall. Therefore, it stands to reason that reduced participation in activities of daily living (ADL), both in and out of the home, may result from challenges with balance and functional mobility (Gill, Allore, Hardy, & Guo, 2006).…”
