The effect of anaesthetic agents on induction, recovery and patient preferences in adult day case surgery

Abstract: Differences in outcome between the four regimens are transient; sevoflurane is not an ideal sole agent for adult day case anaesthesia and, in this setting, patients base their preferences for future anaesthetics on the method of induction.

“…These patient specific modulating variables will be discussed in Section 3 (see below). Based on clinical studies, the primary causes of PONV after surgery are the use of inhalational anesthesia (Apfel et al, 2012a; Apfel et al, 2002; Hofer et al, 2003; Moore et al, 2008; Raeder et al, 1997; Vari et al, 2010) and opioid analgesics (Apfel et al, 2012a; Binning et al, 2011). Figure 1 illustrates the potential stimuli that contribute to PONV and postdischarge nausea and vomiting (PDNV).…”

“…13 However, in other previous studies, another anesthesia method was requested by 22% of patients who received tidal breathing induction with sevoflurane, 14 and even 7 days after surgery, by 43% of patients who received sevoflurane with nitrous oxide. 21 Patients with severe anxiety against dental treatments, like the subjects in this study, probably need general anesthesia for future dental treatment. Selection of a preferred anesthetic is important for such patients, and 80% of the subjects in this study selected propofol as their preferred anesthetic.…”

“…12,14,[36][37][38][39][40][41] One study used a 0-to 10-cm VAS but converted those findings to yes or no (less than 1 cm was counted as no nausea). 42 Only two measures of PDNV have been reported in published studies, the AS-INVR 22 and FLIE. 26,43 The frequency of vomiting was not reported consistently in studies.…”
