The effect of fractal-like mechanical ventilation on vital signs in a rat model of acute-on-chronic liver failure

Abstract: The network of interactions between different organs is impaired in liver cirrhosis. Liver cirrhosis is associated with multi-system involvement which eventually leads to multiple organ failure. This process is accelerated by a precipitating factor such as bacterial infection which leads to respiratory distress, circulatory shock, neural dysfunction and very high mortality. Cirrhotic patients often have blunted respiratory sinus arrhythmia and impaired cardio-respiratory variability. Fractal-like mechanical ve… Show more

“…These, among other rising questions regarding physiological connections, further underpin the aim of network physiologic methods such as functional and parenclitic network analyses (see Appendix A ). This connected role of the liver makes it a good model for network physiology, and continued research along this line is likely to open new frontiers of understanding regarding the critical communication axis or nodes that could be best pharmacologically or nonpharmacologically (e.g., through nerve stimulation, fractal‐like ventilation; Nataj et al., 2018 ) targeted to improve the prognosis of patients with decompensated cirrhosis.…”

“…However, recent reports have revealed more robust therapeutic strategies that have the potential to restore HRV in experimental cirrhosis. Notably, experimental studies have revealed that alteration of the pattern of respiration via fractal‐like mechanical ventilation, in a rat model of ACLF post endotoxin injection, has been associated with increased oxygen saturations and prevented the reduction in HRV, specifically in short‐term HRV indices (Nataj et al, 2018 ). This was concomitantly documented with decreased liver injury and importantly a decrease in death rate.…”

“…Recent studies have suggested the bile duct obstruction Wistar rat model followed by the i.p. injection of a single dose of LPS (1 mg/kg, extracted from Salmonella typhimurium )[ 80 ] or bile duct-obstructed SD rats followed by an i.p. injection of LPS (0.03 mg/kg BW).…”

“…An increase in plasma enzyme levels and splenomegaly were observed, mimicking pathological events and inducing the inflamassome ( e.g. IL-1β; IL-18), as noted in the human syndrome[ 62 , 80 ]. Although these methodologies result in similar conditions to the human disease, the need for surgical procedures and animal survival maintenance make them more challenging and costly, requiring previous surgical knowledge.…”
