The Effect of HES (130/0.4) Usage as the Priming Solution on Renal Function in Children Undergoing Cardiac Surgery

Abstract: Background: Experience with hydroxyethyl starch (HES) in children is limited. This study was conducted to observe the effects of HES or Ringer's lactate (RL) usage as the priming solution on renal functions in children undergoing cardiac surgery. Methods: After ethical committee approval and parent informed consent, 24 patients were included in this prospective, randomized study. During cardiopulmonary bypass (CPB), Group I received RL and Group II received HES (130/0.4) as priming solution. Serum creatinine, … Show more

“…However, a review carried out by Van der Linden et al (9) on 59 primary full publications of studies regarding the safety of HES during surgery did not reveal any adverse safety findings for the use of HES during or immediately after surgery in adults. These results are in line with pediatric findings, which suggest renal function test results in children after HES infusions are at least comparable to the results for the control groups even after liberal colloid substitution (35-50 mlÁkg À1 ) (16)(17)(18). In addition, a prospective multicenter observational study did not detect significant renal side effects in a large cohort of 1104 children and concluded that moderate doses of HES appeared to be safe in children (8).…”

“…One major factor inducing kidney injury during cardiac surgery is bypass‐related renal ischaemia. During bypass, renal vasoconstriction combined with haemodilution decreases oxygen delivery to the kidneys; further impairment occurs due to increased renal oxygen consumption, as well as possible embolisation after weaning from bypass . Haemodilution during bypass can result in decreased oncotic pressure and interstitial oedema; this is compounded when large fluid volumes are administered in order to maintain intravascular volume.…”

“…Very recently, Akkucuk et al [12] documented their results on use of HES 130/0.4 on pediatric patients undergoing CPB and revealed no adverse effects on renal functions and other events.…”
