“…COVID-19大流行期间,重精患者的照料者面临着更加巨大的挑战,他们需要将患者送至医院就医,承担着就医途中感染COVID-19的风险,照料患者及自身患病风险的双重压力导致个体的焦虑抑郁水平比以往更加突出。受疫情的影响,基于网络的心理干预逐渐兴起 [ 23 ] 。本研究实施的时间段正处于COVID-19大流行期间,考虑到外界环境以及照料者时间、空间的限制,选用了基于互联网的MBSR干预模式。在以往标准的8周MBSR基础上,充分听取具有网络教学经验专家的经验,将本次课题的MBSR干预进行了部分改良:①根据照料者的特殊性,补充了重精的发病机制、预后及转归的部分宣教,以帮助照料者减缓照料的耻感;②正念躺式瑜伽练习和站式瑜伽练习除了听取引导语,还需要模仿带领者的动作,均录制了相关的视频,便于学员在课后观看视频,进一步巩固练习效果,以便学员之间练习的同质性;③课后练习情况需要学员在群内打卡。本文资料显示,8周后干预组的SAS、SDS分值较基线时显著降低,且趋于文献报道的正常对照人群的分值[SAS为(29.78±10.07)分,SDS为(41.88±10.57)分] [ 10 , 16 ] 。关于MBSR为何可以缓解个体的焦虑、抑郁水平,不同研究领域的学者的见解并不一致。心理学界认为:①正念练习让个体在觉察当下情绪及情绪背后的原因时,带着自我包容及接纳的态度,而不是评判其好或坏、对或错 [ 24 ] ;②正念不再试图去压制和抵抗焦虑、抑郁情绪的产生,这并不是说遇到事情时必须听之任之、听天由命,也不是说放弃任何为改变处境做出的努力,而是先着眼于自己容易做出改变的内容,去尝试与这些不良情绪共处的过程 [ 25 ] ;③正念练习可以帮助个体更加处于当下,增强觉察能力,觉察清晰后在力所能及的范围内做出改变,培养个体稳定的心境,以此达到缓解焦虑、抑郁情绪的效果 [ 26 ] 。脑科学的学者却认为:①正念练习中身体扫描练习可以通过影响中枢神经系统的可塑性(如增加海马体和腹侧/背侧前额叶的体积)改善静息状态网络,对认知表现间接提供积极的、有价值的影响,促进不良情绪的缓解 [ 27 - 28 ] ;②正念练习中瑜伽练习使个体处于开放、扩张的姿势,提高大脑中的睾酮浓度,从而提升个体的自信心,使其有能力面对问题、解决问题 [ 29 ] 。…”
