The Effect of Induction Method on Defibrillation Threshold and Ventricular Fibrillation Cycle Length

Abstract: The DFT and the VFCL proved to be independent of the VF induction method. The T-wave shock was more unlikely to induce VT during DFT testing. These results suggest that both methods are reliable in DFT determination, though T-wave shock application is a more reliable method for DFT testing.

“…Many studies have focused on the influence of amiodarone on DFT, but the results have been somewhat conflicting. Some authors found that chronic oral amiodarone therapy had no significant effect on DFT in animal studies [18] or clinical studies [12,[19][20][21][22][23], as well as acute amiodarone loading did not change the DFT in animal studies [24]. However, other authors found that chronic oral amiodarone significantly increased DFT in animals [25][26][27] and patients [7,8,11,15,17,21,[28][29][30][31].…”

“…Prior work has shown that human VF is similar between pacing and shock-induced episodes, 15,33 suggesting that clinical VF may exhibit conserved mechanisms with VF induced at electrophysiology study. Based on the success of the animal study results, we proceeded with the first human targeted VF rotor site ablation, which was effective in suppressing recurrent episodes in a patient with clinical VF refractory to antiarrhythmic medications.…”

“…However, it remains controversial whether the induction method has an effect on VF cycle length and defibrillation efficacy. 21 In this study, the DFT was determined using a protocol guided by the ULV. The ULV is defined as the weakest shock strength that fails to induce VF when timed to the vulnerable period of the T wave.…”
