Objective Different drugs are used to treat mastalgia, such as danazol and bromocriptine, and both are associated with side effects, due to which most of women and healthcare providers are interested in herbal medicines. Therefore we aim to study the effectiveness of phytoestrogens on the severity of cyclic mastalgia. Methods To carry out the present study, English electronic resources such as the Cochrane Library, ISI Web of Science, Scopus, and PubMed were used systematically and with no time limitation up to February 10, 2020. Results In total, 20 studies were included in the present meta-analysis. The results of the meta-analysis showed that herbal medicines versus the control group (standard mean difference [SMD] = - 0.585; 95% confidence interval [CI]: - 0.728–- 0.44; heterogeneity; p = 0.02; I2 = 42%), herbal medicines versus the B group (SMD = - 0.59; 95%CI: - 0.75–- 0.44; heterogeneity; p = 0.03; I2 = 42%), and its subgroups, such as phytoestrogen (SMD = - 0.691; 95%CI: - 0.82–- 0.55; heterogeneity; p = 0.669; I2 = 0%), Vitex-agnus-castus (SMD = - 0.642; 95%CI: - 0.84–- 0.44; p < 0.001; p = 203; I2 = 32%), flaxseed (SMD = - 0.63; 95%CI: - 0.901–- 0.367; p = 0.871; I2 = 0%), and evening primrose (SMD= - 0.485; 95%CI:- 0.84–- 0.12; p = 0.008; heterogeneity; p = 0.06; I2 = 56%] may have effective and helpful effects on improving cyclic breast mastalgia. Also, chamomile, isoflavone, cinnamon, and nigella sativa significantly reduced mastalgia symptoms. Conclusion Herbal medicines and their subgroups may have effective and helpful effects on improving cyclic breast mastalgia. The findings of our meta-analysis must be done cautiously because low methodological quality in some evaluated studies of this systematic review.

Background: The prerequisite of effective nutrition intervention is an appropriate education. This study aimed to evaluate the effectiveness of nutrition education based on discussion and multimedia in the learning of pregnant women. Methods: This randomized educational trial was conducted on 174 pregnant women in Gonabad City, northeast Iran. The study participants were randomly assigned into three groups (58, 57, and 59 women in the multimedia group, discussion group, and control group, respectively). The demographic information form and assessment of learning questionnaire were completed before the study for all three groups. In the intervention group, training was based on the discussion and multimedia packages. Multimedia packages were given to mothers in the intervention group for 2 weeks. In the discussion group, mothers were divided into subgroups of 8 to 12 members, and educational subjects were presented over three 90 min sessions. One month after the intervention, a post-test was administered to all groups. Results: The mean gestational ages in the multimedia group, discussion group, and control group were 9.66, 9.54, and 9.40 weeks, respectively. The results showed that the difference between the multimedia and discussion groups (P<0.001) regarding the cognitive scores was significant before and after the intervention. There were significant differences between groups regarding affective domain scores before and after the intervention (P=0.03). Conclusion: The effectiveness of multimedia learning in the cognitive domain and discussion in the affective domain was confirmed. Therefore, it is recommended to use innovative and effective methods such as multimedia packages for educating pregnant women.

Job adjustment predictive factors of healthcare midwives in health system reform in Iran

Background Having sensitive duties and multiple responsibilities in the country's health system, especially after the implementation of the health reform in Iran, midwives need to be able to perform their duties optimally in their new job position as healthcare providers. However, it is not clear to what extent the midwives have adjusted to their new roles and related duties since the implementation of this plan in the country. The aim of the present study was to determine the predictive factors of job adjustment of healthcare midwives in Iran. Methods In this cross-sectional analytical study, 310 midwives were selected from 209 health centers of West Azarbaijan province in Iran and entered the study by census method to answer the research questionnaires. Data were collected using job adjustment, job satisfaction and organizational commitment scales. For data analysis, SPSS software version 25 was used to calculate coefficients of ANOVA and multiple linear regression tests. Moreover, to determine predictive variables, path analysis was used by AMOS software. Results The mean age of the participants was 37.67 ± 7.1 years. Most of the participants (35.5%) were interested in their occupation as a midwife, and mostly had a medium to high tendency (76.1%) to stay in their new profession. Additionally, 58.1% of the participants had moderate job adjustment. "Desire to stay in the midwifery profession" and "organizational commitment" were important predictors of job adjustment for healthcare midwives. “Desire to stay in the midwifery profession”, with a direct effect, and “interest in the new profession” with an indirect effect affected the job adjustment of midwives. Moreover, “adequacy of income to expenses”, “job satisfaction” and “organizational commitment” through the mediating role of the “desire to stay in the profession” can directly and indirectly affect their job adjustment. Conclusion Based on the results of this study, in order to make midwives more adjusted to their role as healthcare provider midwife, organizational managers should focus their efforts and planning mainly on providing incentives to increase the longevity of staying in the profession of midwifery in order to increase job adjustment, job satisfaction and organizational commitment, thereby, improving the quality-of-service delivery.
