Objective. To investigate the anesthetic effect of anterior serratus muscle plane block under ultrasound guidance in arthroscopic shoulder surgery and its effect on postoperative analgesia. Methods. A total of 94 patients who received arthroscopic shoulder surgery in our hospital were selected as the research subjects, and they were divided into a control group and a study group according to the random number table method. The control group underwent brachial plexus block under ultrasound guidance, and the study group underwent anterior serratus plane block under ultrasound guidance. The visual analogue score (VAS) of pain at each time point, intraoperative anesthetic dosage, the use of postoperative analgesic, pain mediators and oxidative stress factors before and after surgery, and the occurrence of adverse reactions were compared between the two groups after surgery. Results. At 6, 12, 48, and 72 h after surgery, there was no obvious difference in the VAS score between the two groups ( P > 0.05 ); 24 h after surgery, the VAS score of the study group was significantly lower than that of the control group ( P < 0.05 ). The intraoperative dosage of propofol and remifentanil in the study group was significantly lower than that in the control group ( P < 0.05 ). The number of effective compressions of analgesic pump and the total amount of sufentanil in the study group were significantly lower than those in the control group within 48 hours after operation, and the time of first compressions of analgesic pump was significantly longer than that in the control group ( P < 0.05 ). Before surgery, there were no significant differences in CGRP, NPY, and MDA levels and SOD enzyme activity between the two groups ( P > 0.05 ). After surgery, the levels of pain mediators CGRP and NPY in the study group were lower than those in the control group ( P < 0.05 ). The level of MDA was lower than that of the control group, and SOD enzyme activity was higher than that of the control group ( P < 0.05 ). There was no significant difference in the incidence of adverse reactions between the two groups ( P > 0.05 ). Conclusion. Ultrasound-guided anterior serratus plane block for arthroscopic shoulder surgery has a good anesthesia effect and long action time. It can significantly reduce the dosage of opioids, enhance the postoperative analgesic effect, effectively inhibit postoperative pain and oxidative stress reaction, and has good safety.

Introduction. Dexketoprofen(DEX) belongs to nonsteroidal anti-inflammatory drugs (NSAIDs) and has analgesic, anti-inflammatory, and antipyretic properties. DEX is an enantiomer of ketoprofen (S+) and has a stronger effect than ketoprofen. It is highly effective even after the administration of small doses. The therapy with DEX does not cause serious side effects and is additionally tolerated by the body. Aim. The review aimed to find original scientific publications on DEX in recent years and its therapeutic efficacy, safety, and tolerability. Method. A systematic review of scientific articles published no earlier than 2015 was carried out. Materials from the PubMed, Google Scholar, and Medline Complete databases were used. The literature review was carried out in November 2021. Among the publications found, 28 scientific articles were selected for analysis. Results and discussion. Over the recent years, there have been many publications about DEX. Articles describing new data on DEX in the treatment of pain were analyzed, compared with other drugs and mesotherapy, the latest reports of its combination with tramadol and thiocolchicoside were reviewed, and a new slow release form of DEX and new therapeutic applications of this drug were investigated. After analyzing all the studies, it was found that DEX produced similar or more effective analgesia compared to other drugs and reduced the need for emergency medications. In addition, it was noted that using DEX in combination therapy was far better than taking it alone, and was also found to be effective in raising the epileptic threshold. Mesotherapy achieved higher results in the treatment of pain symptoms compared to DEX. The side effects that appeared as a result of the use of DEX therapy were not life-threatening. Conclusion. The results of the review confirm that DEX is a very good analgesic, which is more potent than paracetamol and diclofenac sodium while having similar effects to dexmedetomidine and lidocaine. DEX in combination with tramadol or thiocolchicoside is more effective than when the two drugs are used alone. Scientists are working on the long-acting DEX and are looking for new applications of the drug in epilepsy and oncology.

Одной из первоочередных задач ведения больных в интраоперационном и раннем послеоперационном периоде является соответствующее обезболивание, которое обеспечивает не только гладкое клиническое течение заболевания и качество жизни пациентов, но и успех хирургического лечения в целом благодаря снижению количества стресс-реализованных осложнений. Длительная послеоперационная острая боль может быть причиной развития функциональных нарушений со стороны легких, сердечно-сосудистой системы, желудочно-кишечного тракта, центральной нервной системы, системы гемостаза и др. Применение опиоидов увеличивает количество осложнений в раннем послеоперационном периоде. Поэтому сегодня перспективным направлением является применение схем мультимодальной аналгезии, что способствует лучшему обезболиванию, более гладкому клиническому течению заболевания, ранней мобилизации больных благодаря минимизации количества осложнений, вызванных опиоидами. Выбор адекватной модели послеоперационного обезболивания должен основываться на понимании многоплановости болевого синдрома, который может включать как соматический компонент, обусловленный хирургической травмой, так и нейропатический, вызванный хроническим воспалением, нейропатией, ишемическими расстройствами. В статье приведены данные современных исследований относительно концепции мультимодальной аналгезии, которые показывают, что применение в ее составе декскетопрофена (Декспро) способствует более качественной и длительной аналгезии, предотвращает развитие хронической послеоперационной боли, уменьшает потребность в применении наркотических анальгетиков.
