Objective: This study aimed to investigate possible ototoxicity associated with topical rifamycin application via electrophysiological tests and ultrastructural examinations. Methods:Electrophysiological assessment was performed with tympanometry, auditory brainstem response (ABR), and distortion product otoacoustic emission (DPOAE) measurements. This study was conducted on 40 ears of 20 guinea pigs that were detected to have normal hearing thresholds. The animals were randomly assigned to three groups: Group 1 (n=12) received 0.1 mL rifamycin, Group 2 (n=8) received 0.1 ml gentamycin, and Group 3 (n=20) received 0.1 mL physiological saline. The antibiotics and saline solutions were administered via intratympanic injections. After five injections every other day, electrophysiological tests were performed again on the 15th day. After electrophysiological measurements, the temporal bones of all guinea pigs were prepared for ultrastructural examinations and the cochlear surface morphology was examined by scanning electron microscopy (SEM). Results:The animals in group 3 did not show a statistically significant change in their DPOAE signal/noise ratio (SNR) or ABR thresholds (p>0.05). In groups 1 and 2, the reduction in the DPOAE SNR and the increase in the ABR threshold were statistically significant (p<0.05). Regarding SEM examination results, the animals in groups 1 and 2 showed statistically significant outer hair cell damage and cochlear degeneration due to the ototoxic effect of the drugs (p<0.05), whereas the animals in group 3 showed no significant damage (p>0.05). Conclusion:The results indicate that rifamycin application to the middle ears of guinea pigs has mild ototoxic effects on their inner ears.

OBJECTIVE:The aim of this study is to investigate the early histopathologic effects of Burow's and Castellani's solutions on the middle ear mucosa of rats. MATERIALS and METHODS:The study was conducted with 26 Wistar albino female rats. Gelfoam that was soaked in 4% Burow's solution was inserted into the middle ears of the rats in the Burow group (n=10); over 2 weeks, 0.1 mL Burow's solution was administered once a day through perforation into the middle ear. The same procedure was applied to the rats in the Castellani group (n=10) using classical Castellani's solution and to the rats in the control group (n=6) using physiological saline solution. At day 1 after the last administration, all groups were decapitated; their bullas were dissected. The bullas were histopathologically evaluated and graded with respect to increase in leukocytes with polymorphic nuclei, mononuclear cell infiltration, and fibrosis. The data obtained were statistically analyzed. RESULTS:In the Burow group, the fibrosis scores were significantly higher than those in the control group (p=0.039), the scores of leukocytes with polymorphic nuclei were significantly higher than those in the control group (p=0.034), and the total scores were significantly higher than those in the control group (p=0.022). CONCLUSION:We suggest Castellani's solution as a safe alternative in the treatment of otomycosis and external otitis in the presence of tympanic membrane perforation. However, because of the inflammatory changes it causes in the middle ear mucosa, we do not recommend the use of Burow's solution in the presence of tympanic membrane perforation.

Clinical characteristics of granular myringitis treated with castellani solution

Aim of the study is to observe clinical manifestations of granular myringitis (GM) treated with castellani solution. Twenty-four patients (18 women and 6 men; mean age, 49.7 years; range, 9-77 years) diagnosed with GM and treated with castellani solution were enrolled retrospectively. Topical treatment using castellani solution with or without curettage of a granular lesion and serial otoendoscopy-aided photographic documentation of the tympanic membrane were performed. Outpatient follow-up period ranged from 3 to 72 months and the mean follow-up period was 14.4 months. GM was classified into marginal/non-marginal type and ulcerative/polypoid subtype using a new scale. According to the scale used in this study, the marginal and ulcerative GM was the predominant type of GM lesion. Bacterial cultures of ear discharges from 19 patients revealed Staphylococcus, Corynebacterium and Providencia stuartii in 12, 5, and 1 patient, respectively and no growth of bacteria in one patient. Of 24 patients, 23 showed complete resolution of GM after treatment with castellani solution. However, one patient had a persistent, small, dry, deepithelialized lesion and eight others required more than 3 months for complete recovery owing to a persistent residual lesion. There was recurrence in two patients but the lesion resolved after additional castellani treatment. Topical application of castellani solution is a simple and effective treatment for GM. This study suggests a potential of castellani solution for treatment of GM with a serial photographic documentation and novel classification scale for description of lesions. A sufficiently long follow-up period is essential to confirm a complete recovery of GM.
