The effect of various blood management strategies on intraoperative red blood cell transfusion in first-time coronary artery bypass graft patients

Abstract: Effective blood management during cardiac surgery requires a multifactorial effort to limit exposure to allogeneic blood products. The present study evaluated the distribution of intraoperative interventions in patients undergoing coronary artery bypass graft surgery with cardiopulmonary bypass. Records from patients undergoing non-reoperative surgery at 120 hospitals between January 2017 and December 2017 were reviewed, and red blood cell transfusion quartiles established. The 31 hospitals with the lowest tra… Show more

“…A sampling of case records for each clinician is undertaken on a quarterly basis to validate the ICS process and to assess the accuracy of the data. Although the data are primarily used for reporting and performance improvement, it also have been incorporated for prospective research (14,15). It became clear that the trend in utilization for ICS was changing over the past several years with 21,811 procedures in 2016 and 20,811 in 2019, representing a 5% decline over that time (unpublished).…”

“…Consecutive surgical procedure ICS data from the Spe-cialtyCare Operative Procedure rEgistry (SCOPEÔ), between January 2016 and September 2019 at 215 hospitals throughout the United States, were reviewed. The SCOPE was established in 2011 as a national quality control process for systematically collecting intraoperative data from surgical procedures and has been described elsewhere (14,15). It serves a multifunctional purpose focused on quality enhancement and performance improvement.…”

“…In this issue of Perfusion , we publish work by Stammers and co-workers 1 on strategies to reduce transfusion. They analysed data from 120 hospitals and looked in detail at over 5,000 cases from the institutions most restrictive and most liberal with blood to determine associations with transfusion.…”
