The effectiveness of 10% chlorhexidine varnish treatment on dental caries incidence in adults with dry mouth

Abstract: The difference between the 10% chlorhexidine varnish and placebo treatments is considered to be highly clinically significant for root caries increment (41% reduction) and for total caries increment (25% reduction) but only for coronal caries increment (14%).

“…However, gingival recession caused by normal aging, periodontal disease or traumatic toothbrushing habits renders the retained dentition susceptible to root surface caries [2][3][4] . Additionally, the risk for root surface caries is further increased with reduced salivary flow or xerostomia due to aging or medication 5) . According to the literature, the prevalence of root surface caries amongst noninstitutionalized people over the age of 60 years in Western countries and Japan is higher than 50% 4) .…”

“…Because cementum has half as much mineral as enamel by weight, demineralization on the root surface is approximately twice as rapid as that on enamel and can occur at a higher pH 3) . Additionally, root dentine surfaces may even present a high risk to caries development 4) due to a low salivary flow, which is frequently found in elderly as a side-effect of daily use of medicines 5) . Various treatment modalities have been proposed for the management of root caries, including the improvement of oral conditions for oral hygiene, topical application of remineralization agents and antimicrobial regimen 6) , and coating or sealing by resin-based materials 7) .…”
