Background: Study was conducted to analysis efficacy of compression suture in the surgical management of Atonic PPH. It’s prospectively carried out with 60 cases required compression suture in managing Atonic PPH. Atonic uterus is preventable cause of maternal mortality and morbidity constituting 80% PPH cases.Methods: Prospective study carried out at tertiary care center for studying efficacy of compression suture in the surgical management of atonic PPH for one year, started after approval from ethical committee.Results: Compression suture is effective in managing atonic PPH. out of 60 cases, 4 patient's required obstetric hysterectomy as a last resort to control Atonic PPH. Uterus was conserved in 93.3% of cases.Conclusions: Development of compression suture technique has proved to be effective in the control of massive Atonic PPH not responding to medical management is an alternative to hysterectomy when timely applied. It proves to be valuable addition for surgical treatment of PPH and for saving uteri after medical management of PPH failed. Authors can adopt compression suture as a mid-step before resorting to uterine devascularisation or hysterectomy when medical line of management fails. Resident doctors in teaching institutes must be well trained in this technique.

ABSTRACT… Background: Surgical methods of reduction of bleeding from uterus by inserting compression sutures have been developed to reduce the need for emergency hysterectomy in patients with atonic primary postpartum haemmorhage. Objectives: To determine the efficacy of B-Lynch brace suture in the management of atonic primary postpartum haemmorhage during cesarean section. Study Design: A prospective observational study. Setting: Gynaecology Department, Khyber Teaching Hospital, Peshawar. Period: 1 st January 2016 to 31 st December 2016. Methodology: 14 patients who underwent B-lynch suturing for atonic PPH during cesarean section, nonresponsive to medical line of management. Patient's age, parity, booking status, birth weight of newborn, blood transfusion and effectiveness of procedure was noted. The patients were followed up for 6months for any postprocedural morbidity. Results: In all 14 patients with atonic primary postpartum haemmorhage with failed medical treatment, B-lynch suture was applied. 10(71.42%) belonged to 21-30 years age group. 8(57%) were multiparous and 10(71.42%) were non booked. Birth weight of 9(52.94%) cases was 2-3kg.4-5 units of blood was transfused in 6(42.8%) cases. Most common indication for cesarean section was prolonged labor in 4(28.57%), followed by placental abruption and multiple pregnancy in 3(21.42%) each. Success rate of this procedure was 92.85%. It failed in one patient who ended up in hysterectomy. Patients were followed for 6 months and no postprocedural morbidity was seen. Conclusion: This is a beneficial procedure for surgical treatment of atonic uterus especially in young patients where fertility issues are of concern, and has added advantage of less time of application, less bleeding and lesser skill requirement. Postpartum hemorrhage is the most common form of obstetric hemorrhage and is the leading cause of maternal morbidity and mortality in the world The commonest cause of postpartum hemorrhage is uterine atony. It is a serious obstetrical problem and contributes to 25-43% of all maternal deaths in the developing countries. This suture technique was described by Christopher B-Lynch in 1997 that envelope and compresses the uterus to control life-threatening postpartum hemorrhage. Purpose: To study the effectiveness of B-Lynch uterine compression sutures in the control of PPH. Materials: study includes 30 patients whom developed severe atonic PPH not responsive to pharmacological measures at department of Obstetrics and Gynecology at Aswan University hospital, in the period between March 2018 and December 2019. Methods: A prospective observational study, a total of 30 cases of B-Lynch was performed and results analyzed. Results: The procedure was successful in most of the studied cases (86.7%), as no hysterectomy had been done in these patients. On the other hand 13.3% of the studied women had hysterectomy The causative factors accounting for PPH were Placenta previa (43.1%), followed by Abruptio placenta (23.3%) and prolonged/obstructed labour (13.3%) seventy percent of the studied patients (70.0%) had blood loss in the range 1000-1500 ml, whereas 6.7% had a blood loss of more than 2500 ml Slightly more than one quarter of the studied patients (26.8%) did not need any blood transfusion, whereas one third of the patients (33.2%) required one unit and 20.0% of them needed ≥ 3units blood transfusion. Conclusion: The application of B lynch suture is was a very effective method to control primary postpartum hemorrhage. It is was a simple suture and technique of application could be learned by every doctor with little practice .The suture can be used to conserve uterus, with little cost of procedure.
