The Effectiveness of Extracorporeal Shockwave Therapy for Midportion Achilles Tendinopathy: A Systematic Review

Abstract: Achilles tendinopathy is one of the most common lower limb injuries in both athletes and the general population. Despite the plethora of conservative treatment options available for the management of Achilles tendinopathy, as many as one in four patients will go on to require surgery. Extracorporeal shockwave therapy (ESWT) has emerged as a promising treatment option and has been successful in the management of other common musculoskeletal injuries such as plantar fasciitis. However, the evidence for ESWT in t… Show more

“…As mentioned above, the results of these trials are contradictory. In addition, ESWT is used in different ways to treat tendinopathies [ 35 , 36 ]. For instance, ESWT can be administered either radially or focally, and at varying frequencies or intensities.…”

“…ESWT may also be used for Achilles tendinopathy. [64][65][66] A metaanalysis of 8 randomized trials (442 patients) evaluated ESWT compared with other treatments, including sham ESWT, eccentric training, and other nonsurgical treatments. ESWT was associated with greater improvement on AOFAS score (SMD, 1.35 [95% CI, 0.24-2.41]; P = .01) and VISA-A scores (SMD, 1.53 [95% CI, 0.69-2.37]; P = .0003), although the difference was less than the MCID for the VISA-A.…”

“…Conservative treatment in AT includes medical and physical therapy. Physiotherapy includes electrophysical means and manual techniques [ 7 , 15 , 16 , 17 , 18 , 19 ]. Exercise seems to be the intervention with the highest level of evidence [ 10 , 20 , 21 , 22 ].…”
