Background: Thyroid nodule is a common presentation. The estimated prevalence of thyroid nodules is 5-10% by clinical examination and up to 70% on ultrasonographic (USG) evaluation. Most are benign without any symptoms or cosmetic concerns. Only around 7-15% are found to be malignant. Objective: This study was conducted to assess the association between Thyroid Imaging, Reporting, Data System (TI-RADS) and the corresponding category of Bethesda System for Reporting Thyroid Cytology (TBSRTC) in evaluation of thyroid nodules and assessing risk of malignancy. Methods: This retrospective study evaluated a total of 172 patients with thyroid nodules presenting to out-patient laboratory in AL-Kut in a period of three years, by categorizing them into different categories using Thyroid Imaging Reporting and Data System (TIRADS) by USG and The Bethesda System for Reporting Thyroid Cytopathology (TBSRTC) by fine needle aspiration (FNA). The correlation between TIRADS and TBSRTC was evaluated using SPSS Statistics. Result: By FNA, 93% were benign nodules (category II-III), whereas 7.4% were malignant (category IV-V). By ultrasound 85% were benign (TI-RADS 1-3). The risk of malignancy for TI-RADS1, 2, 3, 4, 5 was 0, 0, 2.9, 38, and 80% respectively. No statistically significant difference between males and females. Overall agreement between the cases by USG and FNA using the TIRADS and TBSRTC respectively was significant (p<0.0001), and the area under the ROC curve was 0.913. Conclusion: Our study observed a substantial agreement between the diagnosis made by TIRADS on ultrasound and TBSRTC on FNA, so using TI-RADS as the initial step in stratification of all thyroid nodules and as the only step in TI-RADS 1 and TIRADS2 nodules, so that only suspicious lesions undergo FNA, can reduce the need for invasive more expensive procedures without risking cancer missing.

Background: Due to its availability, high information content and harmlessness, ultrasound is an extremely effective method of differential diagnosis of thyroid pathology. Despite the sufficient arsenal of diagnostic methods, thyroid biopsy with cytological examination is the «gold» standard in diagnostic algorithms. Most often, fine — needle aspiration biopsy is carried out using disposable syringes of different capacities (5, 10, 20 ml3). The procedure is performed in most cases according to the «Free hand» method, this leads to several problems associated with the peculiarity of the study, primarily with the low vacuum created in the syringe and insufficient collection of cytological material, as well as insufficient practical experience for a novice specialist.Aim: Determination of the range of the possibility of using vacuum aspiration fine needle biopsy under the control of ultrasound navigation in patients with focal thyroid pathology.Materials and methods: On the basis of the Problem Research Laboratory «Diagnostic studies and minimally invasive technologies» of the Smolensk State Medical University, a device for vacuum fine — needle aspiration biopsy was developed. In the period 2019–2020, 780 vacuum fine — needle aspiration biopsy was performed under the control of ultrasound navigation in patients with thyroid nodules. The level of vacuum in the syringe during sampling of the material was — 0,5 bar (- 50 kPa).Results: A comparative analysis of the cytological results obtained with the help of vacuum fine — needle aspiration biopsy and the «Free hand» method shows a decrease in obtaining the category of Bethesda 1 by 3.3% and Bethesda 3 by 10.2%.Conclusion: This indicates the effectiveness and in formativeness of the use of vacuum fine — needle aspiration biopsy in patients with focal thyroid pathology in comparison with the standard «Free hand» method, as well as with vacuum fine — needle aspiration biopsy due to constant visual monitoring and the lack of movement of the syringe piston to create a discharge when taking cytological material, the probability of needle deviation from the biopsy zone is reduced, which increases the effectiveness of the study and facilitates the work of a novice specialist.

Introduction. Diagnosis and treatment of lesional pathology of the thyroid are important problems in Russia and worldwide. Currently, fine-needle aspiration (FNA) is the main technique of differential diagnosis of benign and malignant tumors of the thyroid. In 2023, clinical guidelines of the European Thyroid Association (ETA) on thyroid lesion treatment were published noting possible minimally invasive treatment of benign and malignant tumors of the thyroid requiring repeat cytologic verification. Despite the proven effectiveness of FNA, its informativity and specificity vary between 60 and 100 %. Additionally, this method is associated with high risk of false negative and false positive results. Therefore, there is a need to improve FNA and minimize its disadvantages.Aim. To evaluate the capabilities of vacuum FNA of small lesions using multiparameter ultrasound in diagnosis of thyroid cancer.Materials and methods. At the scientific and practical problem laboratory Diagnostic Studies and Minimally Invasive Technologies of the Smolensk State Medical University, a device for vacuum FNA was developed. Between 2023 and 2024, patients with thyroid lesions of size <10 mm underwent 25 vacuum FNAs. Correlation analysis with 51 FNAs performed between 2020 and 2023 was performed.Results. A decrease in the number of nondiagnostic (category I per the Bethesda system, 2017) and undetermined (category III per the Bethesda system, 2017) results was observed. Comparison of preoperative cytologic conclusions (category V per the Bethesda system, 2017) with the results of postoperative histological examination in patients of the control group showed benign lesions in 2 (50 %) cases which correspond to false negative results. Vacuum FNA did not show this discrepancy. These results demonstrate a decrease in the probability of false negative results and effectiveness of vacuum FNA.Conclusion. Use of vacuum FNA expedites ultrasound-guided biopsy for small thyroid lesions. Due to high negative pressure (up to –0.8 bar), single-step retrieval of cytologic material is performed which leads to a decrease in the number of nondiagnostic and undetermined cytologic conclusions by 11.7 and 1.9 %, respectively.
