Changes in patterns of performance for the cognitive functions of memory, processing speed, and focused attention are expected in old age.Objective:The main goal of this systematic review was to analyze the use of ERP in healthy elderly in studies evaluating the P300 components.Methods:A systematic review was carried out based on recommendations for nursing research on the databases LILACS, PsycINFO, PubMed, SCOPUS and Web of Science.Results:26 studies involving 940 healthy elderly were identified, most of which sought to identify and determine the influence of age on the P300.Conclusion:Although there is consensus in the literature that P300 latency is significantly longer in elderly with psychiatric disorders compared to healthy elderly, it was not possible to conclude P300 associations with gender, education and other cognitive tests.

Objective Visual hyperexcitability in the form of abnormal contrast gain control has been shown in photosensitive epilepsy and idiopathic generalized epilepsies. We assessed the accuracy and reliability of measures of visual contrast gain control in discerning individuals with idiopathic generalized epilepsies from healthy controls. Methods Twenty-four adult patients with idiopathic generalized epilepsy and 32 neurotypical control subjects from two study sites participated in a prospective, cross-sectional study. We recorded steady-state visual evoked potentials to a wide range of contrasts of a flickering grating stimulus. The resultant response magnitude vs. contrast curves were fitted to a standard model of contrast response function, and the model parameters were used as input features to a linear classifier to separate patients from controls. Additionally we compared the relative contribution of model parameters towards the classification using a sparse feature-selection approach. Results Classification accuracy was 80% or better. Sensitivity and specificity both were 80–85%. Cross validation confirmed robust classifier performance generalizable across the data from the two samples. Patients’ relative lack of gain control at high contrasts was the most important information distinguishing patients from controls. Conclusions Individuals with idiopathic generalized epilepsy were distinguishable from the neurotypical with a high degree of accuracy and reliability by a reduction in gain control at high contrasts. Significance Gain control is an essential neural operation that regulates neuronal sensitivity to stimuli and may represent a novel biomarker of hyperexcitability.

Сложные взаимодействия эпилептического очага и системной реакции головного мозга требуют использования комплексного подхода к прогнозированию течения фокальной эпилепсии. Цель исследования -изучение роли интериктальных нейрофизиологических показателей и данных нейровизуализации в прогнозировании течения эпилепсии. Пациенты и методы. Обследовано 82 пациента с фокальной структурной эпилепсией и 82 здоровых участника (контрольная группа). Проведена оценка клинических, психологических, социальных характеристик, данных нейровизуализации и комплексного нейрофизиологического исследования (электроэнцефалография, регистрация экзогенных и когнитивных вызванных потенциалов, моторное тестирование, вариабельность сердечного ритма). Результаты и обсуждение. Выделены прогностические факторы неблагоприятного течения эпилепсии: височная эпилепсия, поражение левой височной доли, нетравматические внутримозговые кровоизлияния. Предложены алгоритмы прогнозирования течения эпилепсии на основании нейрофизиологических и нейровизуализационных данных. Анализ физиологических показателей у пациентов с неблагоприятным течением эпилепсии продемонстрировал замедление основного ритма на электроэнцефалограммах, снижение мощности специфической афферентации и увеличение времени принятия решения в отношении стимула по данным потенциала Р300, недостаточность центральных механизмов обеспечения моторной реакции. У этих пациентов определялось также увеличение активности стресс-реализующих систем. Учет не только нейрофизиологических показателей, но и данных нейровизуализации позволил улучшить прогностические возможности искусственной нейронной сети, определяющей тип течения заболевания. Заключение. Неблагоприятное течение фокальной эпилепсии ассоциировано с рядом клинико-физиологических параметров, при этом возможно выделение специфического физиологического паттерна данного типа течения. Комплексный клинико-физиологический подход и данные нейровизуализации позволяют успешно прогнозировать течение заболевания с использованием технологии машинного обучения.Ключевые слова: фокальная структурная эпилепсия; нейрофизиологические параметры; нейровизуализация; машинное обучение; предикторы течения эпилепсии.
