The Epidemiology of Gonorrhea Among Men Who Have Sex With Men in Stockholm, Sweden, 1990–2004

Abstract: Gonorrhea is a marker for HIV infection in MSM, but the increase in gonorrhea may be associated with genital-oral sexual practice rather than with high-risk sexual practice.

“…Even if figures have changed since the survey was performed it is clear that there is an overrepresentation of MSM among gonorrhoea cases in our study, a result common with other studies performed in similar settings both in Spain (Madrid [13] and Barcelona [14]) and elsewhere [15]. HIV prevalence among MSM was very high, 20.9%, although this figure is intermediate between the 31% found in the United Kingdom (UK) [16] and the 11% found in a Stockholm in studies performed in STI clinics [17]. It is worrisome that most HIV-infected MSM were aware of their status before their gonorrhoea diagnosis, and yet they had not used condoms, a finding also described in other studies [17], which highlights the importance of continuous reinforcement of secondary prevention in MSM infected with HIV.…”

The fact that in our study HIV prevalence is increasing only among MSM gonorrhoea cases adds urgency to this recommendation.…”

“…The advent of HIV and AIDS in the 1980s heralded a change in sexual behaviour which was associated with subsequent falls in the number of STI diagnosed in the early 1990s [27,28]. The resurgence of syphilis and gonorrhoea among MSM in Western Europe coincided with the introduction of highly active antiretroviral therapies (HAART) and the resultant decrease in HIV-associated mortality [29].…”

“…[12][13][14] Most cases of gonococcal keratoconjunctivitis occur in sexually active adults and are transmitted by contact with infected urine or genital secretions, and the increase in gonococcal keratoconjunctivitis seems to be associated with the increase of genital-oral sexual practice. 15 Gonococcal keratoconjunctivitis is a potentially devastating infection because of the ability of N. gonorrhoeae to cause severe, ulcerative keratitis, which may rapidly progress to corneal perforation. [6][7][8] Therefore, it is necessary to obtain an accurate diagnosis and commence parenteral antibiotic treatment as early as possible.…”
