The epidemiology of regional and widespread musculoskeletal pain in rural versus urban settings in those ≥55 years

Abstract: Objectives: To examine whether the prevalence of regional and chronic widespread pain (CWP) varies with rurality and to determine the characteristics of persons in rural locations in whom pain is found to be in excess. Methods: Participants, aged ≥55 years, from participating general practices in seven different geographical locations in Scotland were sent a postal questionnaire. The 1-month prevalence of 10 regional pain conditions plus CWP was identified using body manikins. Differences in the prevalence of … Show more

“…Factors were chosen which were known to be risk factors for pain and were likely to be associated with alcohol consumption. These were: age (years) [21]; Body Mass Index (BMI) [25]; education (having a university degree or not) [19]; deprivation (Townsend Index [24]) [17]; social networks (frequency of visits by family and friends) [3]; mood ('Do you ever feel miserable for no reason? ', and 'Do you often feel fed-up?')…”

“…It, thus, causes a major burden on individuals, health systems, and social care systems, and is expected to become the most important public health challenge with the population aging. The reported prevalence of musculoskeletal pain varies, however, the rate of chronic widespread pain, the most serious category, is rather consistently reported to be around 12% [ 2 ]. Musculoskeletal pain results from multiple, heterogeneous pathologies including osteoarthritis (OA), tendinitis/bursitis and fibromyalgia[ 3 ].…”

“…While some studies of chronic widespread musculoskeletal pain have suggested that there are no differences between urban and rural populations [13], global prevalence of pain conditions, especially in areas like Central America, South America, and Sub-Saharan Africa, is hard to truly estimate due to poor epidemiologic data. This leads to a likely underestimation of the global impact of musculoskeletal pain.…”
