The evolution of extracranial approaches to the pituitary and anterior skull base

Abstract: The development of extracranial approaches to the anterior skull base has been a process in development for over a 100 years. Many neurosurgical (and non-neurosurgical) pioneers have contributed to its evolution. In this paper, we will review the major contributors and contributions to this evolving field.

“…34 However, it wasn't until the technical advances in the 1970s, 1980s, and 1990s that the endoscope became useful and effective as a neurosurgical tool. 34 Neuroendoscopy has emerged as an important adjunct to all types of neurosurgical procedures by extending the visualization of the neurosurgical approach beyond the tunnel vision of the operating microscope. 52 The endoscope provides greater direct illumination with higher magnification in a deep operative field.…”

“…1 This less-invasive approach was shown to be safe, with low morbidity and very low operative mortality. 1,2 However, the relatively recent introduction of the rigid fiberoptic endoscope is suggested to be a further technical refinement in pituitary surgery.…”

“…26 While the trans-sphenoidal route is not new, continuously improving endoscopy and instrumentation have greatly expanded the range of what can be safely attempted through the nasal cavity (Fig 10). 27 In addition to lesions in or adjacent to the sella turcica , lesions of the cribriform plate, planum sphenoidale, clivus, and dens may be accessed. 28 Some surgeons have also used the technique to access paramedian middle and posterior fossa lesions.…”
