The Experience of Pregnancy After Renal Transplantation: Pregnancies Even Within Postoperative 1 Year May Be Tolerable

Abstract: A younger age at transplantation and at pregnancy was associated with a greater likelihood of a live birth. Transplantation to conception interval of less than 1 year was not associated with a greater number of adverse pregnancy events when compared with the group with transplantation to conception interval greater than 1 year.

“…Graft function in the 10 -year period was similar between patients with pregnancy history and controls (12)(13)(14)(15). No significant differences are reported in the mean level of creatinine in pre-and post -pregnancy period, in many studies (12,(18)(19)(20).…”

“…Although graft dysfunction and obstructive uropathy are reported in pregnant patients with kidney transplantation (9), studies compared the long -term outcomes of pregnancy on transplanted kidney with nulliparous transplanted controls did not show significant differences in the function or survival of graft in long term (10)(11)(12)(13)(14)(15)(16)(17) (Table 1). Graft function in the 10 -year period was similar between patients with pregnancy history and controls (12)(13)(14)(15).…”

“…9 However, a meta-analysis of 50 studies reported a shorter time of 38.4 months. 10 Evidence suggests that a period of 1 year after transplant appears to be sufficient to minimize the risk of adverse events due to pregnancy, 11,12 as possible risks of acute rejection and graft loss 3,6,13 and prematurity will be less after this time. 14,15 Adequate graft function before conception (no proteinuria and wellcontrolled blood pressure) is the key factor toward a safe pregnancy because these 2 conditions are associated with poor outcomes to fetus and pregnant mothers after kidney transplant.…”
