The Filtering, Clear-cornea Diathermal Keratostomy

Abstract: MMC IDK seems to be simpler and quicker than the modern trabeculectomy, and with similar success rate and safety. Revision by internal needling is easy and efficient. Thus, MMC IDK may be a valid alternative and may also be recommended after failed MMC trabeculectomy, replacing shunting. Randomized, controlled studies are indicated.

“…In the study by Kessing, success was defined as IOP £18 mmHg, and a postoperative IOP decrease of ‡30% with or without reoperation or recanalization. However, if we compared the two studies using the qualified success criteria (IOP £18 mmHg, IOP lowered ‡30% with no regard to postoperative medication or reoperation), our rate of success in the IDK group was still lower (64%) than the study by Kessing (92%) ( Table 5) (Kessing et al 2008).…”

“…In a Danish multicenter study by Kessing et al (2008) 11 of 12 eyes (92%) with POAG operated with IDK fulfilled the success criteria after a mean follow-up of 14 months. We did not obtain a similar rate of success in the IDK group as observed by Kessing, despite the fact that the preoperative data were equivalent with ours.…”

“…Most of the studies lasted for at least 1 year, with 26 (30%) lasting for 2 or more years. Many of the technologies and surgical techniques were each described by a small number of prospective studies ( Figure 1) including endocyclophotocoagulation (ECP), [7][8][9][10][11] MicroPulse, 12,13 Ultrasound-based procedures, 14,15 YAG gonio procedures, [16][17][18] micro trabecular procedures, [19][20][21][22][23] Diathermy-based procedures, 24,25 Excimer Laser, 26 Kahook Dual Blade (KDB), 27,28 Hydrus, [29][30][31] and CyPass. [32][33][34][35][36] iStent, Trabectome, and Xen had the highest number of prospective studies.…”
