The Freedom Solo pericardial stentless valve: Single-center experience, outcomes, and long-term durability

Abstract: The Freedom Solo stentless aortic valve is safe to implant and shows excellent early and midterm hemodynamic performance. However, SVD was observed in a substantial number of patients after only 5-6 years and the need for explantation increased markedly, suggesting low durability.

“…Relative to stented valves, such valves offer the advantage of combining a non‐obstructive effective orifice area (EOA) with a flexible aortic root that is thought to be essential for natural leaflet stress distribution, thereby limiting valve degeneration and failure . Depending on prosthesis size, the EOA increases two‐ to threefold after FS implantation and has been reported (mean ± standard deviation [SD]) for the valve sizes 19, 21, 23, 25, and 27, as 1.49 ± 0.32 cm 2 , 1.67 ± 0.40 cm 2 , 1.92 ± 0.38 cm 2 , 2.01 ± 0.42 cm 2 , and 2.13 ± 0.36 cm 2 , respectively .…”

“…The design of this prosthesis closely resembles that of the Freedom Solo (Sorin Group) stentless prosthesis that was associated with a higher incidence of SVD at 5 years. 3 However, in this case, the patient's age and history of chronic kidney failure are 2 major risk factors that could explain the early SVD observed. 4 Thus, this report of early Perceval degeneration should be taken with caution.…”

“…Evidence for the longevity of the Freedom SOLO valve is mixed with reported 10 year freedom of SVD ranging from 92% to as low as 70%. 62,66,67 As such, the current evidence suggests that the longevity of the FS salve is inferior to that of commonly used stented valves, particularly that of the Edwards Perimount.…”
