The gentamicin-collagen implant and the risk of distant metastases of rectal cancer following short-course radiotherapy and curative resection: the long-term outcomes of a randomized study

Abstract: PurposeA previous randomized study conducted by our group showed that application of gentamicin-collagen implant (GCI) into the pelvic cavity after total mesorectal excision (TME) reduced the incidence of distant metastases. Therefore, we decided to conduct a confirmatory study.MethodsPatients with rectal cancer were included in the study if they met the following criteria: adenocarcinoma of the rectum, preoperative short-term radiotherapy (5 × 5 Gy), and WHO performance score 0–1.ResultsOne hundred seventy-si… Show more

“…observed in a randomised trial a significantly reduced rate of distant metastases in patients with GCS after TME with preoperative radiotherapy compared to a control group without GCS 21. These surprising findings were also demonstrated in a confirmatory study from the same institution with the median follow-up of 80 months 36. These results are difficult to explain, in particular, the fact that there were no significant differences in overall and disease-free survival rates.…”

“… 21 These surprising findings were also demonstrated in a confirmatory study from the same institution with the median follow-up of 80 months. 36 These results are difficult to explain, in particular, the fact that there were no significant differences in overall and disease-free survival rates. Apart from the assessment of oncological outcomes, a detailed cost-effectiveness analysis as well as a long-term evaluation of conceivable late side effects (anastomosis stricture, persistent anterior resection syndrome) are other interesting topics worth to be explored in the future.…”

“…The objective of the first study (Nowacki et al) [1] was to determine the effect of GRM on the risk of post-operative complications and oncological results. The surprising results of the 3-year long-term follow-up, showed a significantly lower rate of distant metastases in the group of patients operated on with the use of GRM inspired another study with the objective to confirm the previous results (Rutkowski et al) [2]. This time, the main objective of the project was to confirm the anti-cancer properties of GRM with regards to a reduction of generalized relapse.…”

“…The analysis concerned the data of the patients who participated in two randomized studies, completed and published. The criteria of participation were described before [1,2]. Both studies concerned patients with rectal primary adenocarcinoma.…”
