The Golden Compass Program: Overview of the Initial Implementation of a Comprehensive Program for Older Adults Living with HIV

Abstract: The population with HIV is aging and has unique health needs. We present findings from an evaluation of the geriatric-HIV program, Golden Compass, at San Francisco General Hospital. We used the implementation science framework, RE-AIM ( Reach, Effectiveness, Adoption, Implementation, Maintenance) to guide the evaluation and used quantitative and qualitative methods to assess RE-AIM dimensions. From January 2017 to June 2018, 198 adults age ≥50 years participated in the program, with an estimated reach of 17%. … Show more

“…Few integrated care programs for older people with HIV have been evaluated. The findings in this and in previous assessments of Golden Compass [ 15 ] are consistent with the limited literature demonstrating high patient and provider satisfaction with care models that incorporated geriatric medicine in HIV care [ 22 , 23 ]. Principles of geriatric medicine emphasize a comprehensive problem-solving approach to sustaining functional ability and improving quality of life [ 22 , 23 ].…”

“…There is some progress in the clinical use of the CGA, which often includes geriatric referral and geriatric syndrome assessments. 128,129 Newer approaches that incorporate artificial intelligence and machine learning to omic and dataset methods in infection and integrative medicine, although early in development, appear promising. 130 None of the composite scores currently capture all the relevant biological processes to provide a holistic understanding of biological ageing either in people with HIV or in the general population.…”

“… 17 Programs to meet needs of older people with HIV are being developed around the world. 18 Examples like the Silver Clinic in England 19 and Golden Compass in San Francisco 20 have described significant comorbidities, functional burdens, and geriatric syndromes. Functional deficits are not irreversible, and timely interventions may be able to improve quality of life.…”
