The Great Imitator: Latent Neurosyphilis Revealed After Initiation of the Immunosuppressive Drug Secukinumab

Abstract: Syphilis is a multi-organ system bacterial infection caused by the bacterium Treponema pallidum . Syphilis can advance through four clinical stages: primary, secondary, latent, and tertiary. Once in the tertiary stage, mortality is seen in up to 58% of individuals. Here, we present a case of latent neurosyphilis manifesting after initiation of the immunosuppressive medication secukinumab, a monoclonal antibody that antagonizes interleukin-17A. A 66-year-old male with type II d… Show more

“…However, before reaching diagnostic closure, infectious causes should also be fully considered, especially given the patient's immunocompromised status. Both primary infection and reemergence of latent infections are a possibility, with rare cases of reemergence of latent syphilis after initiation of immunosuppressive medications being reported [5,6].…”

“…All patients on anti-TNF therapy had positive serological tests; however, immunosuppression is a risk factor for the accelerated course of the disease, and potentially for neurosyphilis (Supplementary Table 1). 3 , 4 , 5 , 6 , 7 , 8 The confirmation of neurosyphilis, ocular syphilis or otosyphilis requires the intravenous use of crystalline penicillin G 18 to 24 million units per day, or alternatively ceftriaxone 1 g to 2 g daily (intravenously or intramuscularly) for ten to 14 days. 9…”
