The Higher Proportion of Men with Psoriasis Treated with Biologics May Be Explained by More Severe Disease in Men

Abstract: ObjectivesModerate to severe psoriasis, once regarded as merely a skin disease, is today seen as an inflammatory systemic disease. The sex ratio of the prevalence of psoriasis is balanced. In recent years several reports have documented that men receive more systemic or UV treatment than women, and different hypotheses were made. In PsoReg, the national registry for systemic treatment of psoriasis in Sweden, we have, like other European registries, observed a predominance of men (59%), especially of men treate… Show more

“…Male patients may exhibit a distinct biological response to etanercept compared to females, implying a different pathophysiological mechanism of psoriatic plaques. This finding is consistent with a previous report, which suggested that males exhibit more severe psoriasis, as compared with females (40). Additionally, TLR-2 and -9 may play a role as indices of severe psoriasis, since their mRNA alterations showed significant positive correlations with ΔPASI-2 in male patients that received etanercept.…”

“…The exact cause of this disease is not fully understood yet (5). It is equally prevalent in men and women (1), affecting 1% -3% of the population worldwide (6), 3% of the American population (2), and 9% of the Iranians population (7). Moderate to severe psoriasis is associated with diseases, such as cardiovascular diseases, metabolic syndrome (1), anger, anxiety, frustration (5), increased risk of mortality (8), diabetes, hypertension, dyslipidemia (9), obesity (3), and reduced quality of life (10).…”

“…4 It may be explained by gender based differences of both innate and adapted immune responses in immunological diseases like psoriasis. 5 Increase in severity of psoriasis in manual labourers may be due to an increased chance of trauma and subsequent koebnerisation in manual labourers.…”
