The highest prognostic impact of LDH among International Prognostic Indices (IPIs): an explorative study of five IPI factors among patients with DLBCL in the era of rituximab

Abstract: Although the International Prognostic Index (IPI) is considered as the current standard prognostication system for diffuse large B-cell lymphoma (DLBCL), prognostic heterogeneity is suggested to exist among the patients within the same IPI risk group. Hence, we investigated the pattern of distribution and prognostic impact of five IPI factors within the same IPI score. We retrospectively reviewed the medical records of 387 patients newly diagnosed as pathologically proven DLBCL between February 2002 and Februa… Show more

“…Although most of our patients (86%) had a low ECOG performance status (a prognostic sign well recognized to be good), 84% had a high LDH level. A high LDH level has been found to be an adverse prognostic factor even in the rituximab era . However, in a univariate analysis of our patients, the LDH level was not found to be associated with OS or PFS.…”

“…Serum IgD level correlated with serum β 2 -MG and LDH levels. Many published studies have demonstrated that serum β 2 -MG and LDH levels are associated with the prognosis of DLBCL [36,37] and IPI is a well-established prognostication system for risk stratification of DLBCL [38]. We also found mIgD and IgDR expression was more common in the GCB type of Hans classification.…”

“…Additionally, elevated LDH levels are independently predictive of adverse clinical outcomes. 32,33 To compare the performance of Ig-HTS detection of disease from plasma or circulating cells, we assessed blood draws from patients with active disease (time of initial diagnosis or relapse) as measured by PET/CT. At these paired time points, LDH had a sensitivity of 59% (20 of 34) compared with 88% (30 of 34) by plasma Ig-HTS (P 5 .01; Figure 2B).…”
