“…Moreover, the same group reported that adjuvant AV fistula does not improve the patency of femorocrural PTFE bypass with distal vein cuff. 12 In our study, early bypass occlusions appeared in nearly 50% of patients in the nonfistula group in comparison with 17% in the fistula group, accompanied by a leg salvage rate of 92% in the fistula group and long-term patency of 47%. We speculate that the increased bypass flow by itself is not the key for patency and leg salvage, but rather increased bypass flow in combination with the use of Omniflow prosthesis.…”

“…A series of 8 lower extremity trauma patients has also been reported where a distal, Scribner-type [10] AV shunt was used in vascular reconstruction with good results [6]. Distal AV fistulas are also being studied in the context of decreasing arterial thrombosis in high-risk arterial reconstructions for atherosclerotic disease [11].…”

“…Rather, these improvements are perhaps more likely to result from peripheral adaptations such as improved efficiency of tissue oxygen usage. The finding, by Laurila et al, 1 of no impact on the patency of crural bypasses employing PTFE, an interposition vein cuff and an arterial-venous fistula, requires critical assessment. The authors acknowledge the potential of bias in this small patient sample but several other factors may be operative.…”

“…VO 2 peak is a function of many factors including cardiac and respiratory performance, and peripheral oxygen usage. 1,2 As such, a test which was limited by general fatigue (cardiac and respiratory performance) rather than claudication pain was more suitable to assess cardiopulmonary fitness. A VO 2 peak assessed by treadmill might be limited by claudication symptoms, whereas these were reported in only 10 of the 32 cycle ergometer tests we performed.…”
