KEYWORDSAsthma; activity impairment; healthcare resource use; quality of life; burden of asthma; disease control ABSTRACTObjective: To determine the association of asthma on health-related quality of life (QoL), productivity, and use of healthcare resources among adults in Brazil. Methods: Data were analyzed from the 2015 Brazil National Health and Wellness Survey, a cross-sectional survey with 12,000 adult respondents. Asthma and control groups were compared with respect to health-related QoL, asthma control, work productivity, and adherence. Generalized linear models were developed to compare asthma-related associations controlling for potential confounding factors. Results: Among respondents, 4.1% (n = 494) reported an asthma diagnosis; those without asthma symptoms were used as the control group (n = 11,487). Regarding asthma control, 51.2% of patients had uncontrolled asthma, 36.4% partially controlled asthma, and 12.3% were fully controlled. Short-acting β2 agonists were the most commonly used class of drugs (38.5%). Approximately 32.4% of asthma patients were considered fully adherent to their treatment. In multivariable analyses, asthma patients presented lower health-related QoL and had more frequent visits with medical healthcare providers (6.1 versus 4.2) emergency room visits (1.0 versus 0.5), and more hospitalizations (0.4 versus 0.2), than control respondents six months prior to the study (p < 0.05). Rates of absenteeism and presenteeism varied between 11.5% and 7.4% (p < 0.05) and 30.4% and 20.9% (p < 0.001) between asthma patients and controls, respectively. Conclusions: Asthma had a negative association on health-related QoL, work productivity, and use of healthcare resources. Excessive use of short-acting β2 agonists and poor treatment adherence reflect poor asthma control and suggest the need to implement new strategies for asthma treatment in Brazil.

Social network of children with cronic disease: knowledge and practice of nursing

Objectives:To identify the knowledge and practice of primary care nurses about the social network approach for fa milies of children with chronic diseases. Methods: Qualitative research, conducted by means of interviews with 23 family health nurses, from one municipality in Paraíba and one in Rio de Janeiro, from June to July of 2017. The data were interpreted using thematic analysis. Results: Social network meant institutional support offered by services outside the unit, and socioeconomic problems involved the family context. In practice, nurses find it difficult to provide comprehensive care and establish ties with families. When referring to other professionals, a weakness in the counter-referral to the family health unit is found. Final considerations: Some gaps were found regarding the knowledge and practice of nurses regarding the social network approach, which requires professional training to strengthen social relationships and the necessary support for families of children with chronic diseases.

RESUMO: Objetivo: Comparar a evolução dos indicadores referentes à asma nas edições da Pesquisa Nacional de Saúde do Escolar (PeNSE) 2012 e 2015. Métodos: Estudo transversal em que foram incluídos escolares do nono ano de escolas públicas e privadas das capitais brasileiras. Para saber se o escolar teve chiado no peito nos 12 meses anteriores ao inquérito, foi feita a pergunta: “Nos últimos 12 meses, você teve chiado (ou piado) no peito?” (sim/não). E para saber se teve asma alguma vez na vida foi questionado: “Você teve asma alguma vez na vida?” (sim/não). Resultados: Verificou-se que 23,52% dos estudantes relataram chiado ou piado no peito nos ultimos 12 meses, variando de 16,80% em Salvador (Bahia) a 27,43% em Porto Alegre (Rio Grande do Sul). Ter asma alguma vez na vida foi relatado por 17,92% dos estudantes, variando de 13,98% em Campo Grande (Mato Grosso do Sul) a 30,35% em Porto Alegre (Rio Grande do Sul). Também foi verificada redução da prevalência de chiado (ou piado) no peito nos últimos 12 meses, entre as duas pesquisas (PeNSE 2012 e 2015) em 20 das 27 capitais do Brasil, com destaque para Belo Horizonte, Florianópolis, Cuiabá e Goiânia. Por outro lado, houve aumento da prevalência daqueles que relataram asma alguma vez na vida em 26 das 27 capitais do país. Conclusão: Houve tendência à redução dos sintomas de asma nos últimos 12 meses, enquanto se observa aumento na proporção de adolescentes em que a asma foi referida alguma vez na vida. De certo que o monitoramento da asma ao longo dos anos é imprescindível para gerar conhecimentos e embasar políticas públicas de controle da asma.
