The impact of carotid artery disease on outcomes of patients undergoing coronary artery bypass grafting

Abstract: Objective: Although the overall complication rate has been decreased significantly in recent years, stroke is a severe complication after coronary bypass operations. The purpose of this study is to evaluate the influence of carotid artery disease on the results of patients under CABG operation. Methods: In a retrospective cohort study, 1,978 patients under CABG operation were studied in Shariati Hospital between April 2002 and March 2006. The patients who had only valve replacement or non-CABG procedure were ex… Show more

“…In previous studies [6,8,9,21], the causative mechanisms and risk factors for perioperative strokes in patients undergoing CABG were determined. The incidence of postoperative stroke correlates mainly with the adverse effect of cardiopulmonary bypass due to large numbers of microemboli built by fragments of atheromatous plaques from the ascending aorta, thrombi, platelets, destroyed red blood cells, white blood cells, lipids, and gas [11,13,22].…”

“…These complications significantly increase disability and mortality of the patients, as well as healthcare costs [5,6]. A number of risk factors can contribute to the occurrence of CNS complications after CABG, including age, unstable angina, prior stroke or transient ischemic attacks, carotid bruits, carotid artery disease, left ventricular ejection fraction (LVEF) below 50%, cardiopulmonary bypass, postoperative atrial fibrillation, and postoperative hypotension [6][7][8][9]. Intracranial artery disease is also an independent risk factor for CNS complications after CABG [10].…”

“…Antiplatelet agents are sometimes administered in addition to management of any atherosclerotic disease including hypertension, diabetes mellitus, and hyperlipidemia [1-3]. However, these procedures are not supported by high-grade evidence, and the indications for prophylactic vascular reconstruction surgery depend on individual clinical characteristics [4].…”
