The Impact of Different Surgical Techniques on Outcomes in Laparoscopic Sleeve Gastrectomies

Abstract: LSG is a safe procedure with a low morbidity rate. SLR is associated with increased leak rates. A surgeon should consider risks, benefits, and costs of these surgical techniques when performing a LSG and selectively utilize those that, in their hands, minimize morbidity while maximizing clinical effectiveness.

“…All bariatric and metabolic operations (eg, adjustable gastric band, Roux-en-Y gastric bypass, sleeve gastrectomy) and procedures for complications directly related to these operations performed at accredited hospitals (see supplemental file for more details) are accrued. 16 17 18 19 21 22 Supplemental table 1 depicts the characteristics of MBSAQIP accredited hospitals compared with non-MBSAQIP accredited hospitals.…”

“…The current management of the staple line or anastomotic leaks is a step-wise approach. Non-operative and less invasive measures, such as percutaneous drainage, endoscopic repair of leak, and stent, are feasible, safe, and effective for staple line leaks in patients undergoing LSG [ 21 , 22 ]. In the present study, the same approach was applied and the patient recovered in about six weeks of time post-operatively with no residual complications.…”

“…Gentileschi et al 24 randomized 120 patients to suturing, Floseal (Baxter Medical), or SeamGuard with no difference between the groups in leak rate. Berger et al 25 analyzed the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) database of 189,477 LSGs and showed that the use of SLR is associated with an increased risk of staple line leak.…”

“…Several cohort studies have reported the effect of reinforcement on leak rate. 25 , 26 Noel et al 26 analyzed 2012 LSG cases and reported a significant decrease in leak rate with SLR. Ser et al 27 also reported a positive effect of reinforcement on leak rate, but Stamou et al 28 showed no such positive effect of reinforcement.…”
