The impact of platelet transfusion characteristics on posttransfusion platelet increments and clinical bleeding in patients with hypoproliferative thrombocytopenia

Abstract: Platelet characteristics, such as platelet dose, platelet source (apheresis vs pooled), platelet donor-recipient ABO compatibility, and duration of platelet storage, can affect posttransfusion platelet increments, but it is unclear whether these factors impact platelet transfusion efficacy on clinical bleeding. We performed secondary analyses of platelet transfusions given in the prospective randomized Platelet Dose Study, which included 1272 platelet-transfused hematology-oncology patients who received 6031 p… Show more

“…37 The dose of 3001 billion platelets in such a unit is barely 16% of the total number of platelets normally in the body and was expected to raise the platelet concentration by 28 3 10 9 /L. 38 Nevertheless, the effect associated with this dose was clinically significant.…”

“…However, a requirement for testing platelets for use beyond day 3 would require testing of a much larger percentage of inventory (74% in the PLADO trial 11 ). The time and effort required for inventory management, testing and labeling will be significant.…”
