The Importance of Colonoscopy in Inflammatory Bowel Diseases

Abstract: Introduction: Endoscopic evaluation, particularly the macroscopic mucosal and histological results of ileocolic biopsies, is essential for the management of inflammatory bowel disease. Endoscopic appearance is not always sufficient to differentiate Crohn’s disease and ulcerative colitis, but there are some characteristics that favor one or another diagnosis. Both diseases have an increased incidence of colorectal carcinoma; so, surveillance colonoscopy is important for detecting early neoplastic lesions. Obje… Show more

“…For the appropriate choice of the best therapeutic approach, it is necessary to consider a number of factors, including the degree of clinical and endoscopic activity of the disease, location, extent, behavior, efficacy of the drug and its potential side effects, prior response to some type of treatment, presence of extraintestinal manifestations or disease-related complications, in addition to the cost-effectiveness issue 4 , 13 . In addition, there are available methods to evaluate possible causes of loss of anti-TNF response, but they are high cost, with little availability, making them unviable 17 , especially in our country.…”

“…13,14 Colonoscopy is the gold standard for making the diagnosis of UC by allowing direct visualization of the colon and taking multiple tissue biopsies for histopathologic analysis to confirm the diagnosis. 15 Radiological modalities used for diagnosis and assessment of UC include Computerized Tomography Enterography (CTE) and Magnetic Resonance Enterography (MRE). 16 MicroRNAs (miRNA or miR) are small (about 18-22 nucleotides in length), highly conserved, single stranded, noncoding RNA molecules that bind target miR to prevent protein production.…”
