The incorporated aortomitral homograft for double-valve endocarditis: the ‘hemi-Commando’ procedure. Early and mid-term outcomes†

Abstract: Compared to double-valve replacement with IVF reconstruction ('Commando operation'), the early and mid-term outcomes of the hemi-Commando procedure proved to be a feasible treatment option for IVF reconstruction, enabling preservation of the mitral valve and the subvalvular apparatus in high-risk patients with invasive double-valve IE.

“…[9][10][11] Coplex valvular IE that involves the intravalvular fibrosa, or a perivalvular abscess requires aggressive debridement, complex reconstruction, and a prolonged procedure to eradicate the biofilm-associated infection. 9 In Dr. David's series of 90 patients with double valve IE, the incidence of postoperative pulmonary complications was 24% with associated mortality 9%. 10 In two different reports of invasive IE published by our group, respiratory insufficiency incidence was 39% and 65%.…”

“…This procedure is performed rarely and long-term data are not available. The only publication dealing with mid-term results involves 37 patients operated on between 2010 and 2017 in Cleveland Clinic (4). In-hospital mortality was 8% and 1-and 3-year survival was 91% and 82%, respectively (only one death was due to recurrent IE).…”

“…In such situations, a more aggressive approach is sometimes required. The reconstruction of the fibrous skeleton through the so-called "Hemi-Commando" or "Commando" operations represent a radical option that entails double valve replacement (44,45) .…”
