The influence of customized abutments and custom metal abutments on the presence of the interproximal papilla at implants inserted in single‐unit gaps: a 1‐year prospective clinical study

Abstract: The restoration of single-implants using computer-assisted design/computer-assisted manufacturing abutments appears to help maintaining a regular papillary filling although the variations of the implant positioning or the restoration/teeth relation.

“…None of the selected studies were randomized controlled clinical trials. Three studies were retrospective (Choquet, Hermans, Adriaenssens, Daelemans, Tarnow, & Malevez, ; Cosyn, Sabzevar, & De Bruyn, ; Perez, Segalla, Marcantonio Jr, Lauris, Ribeiro, & Ferreira, ), and six studies were prospective (Borges, Lima, Carvalho, Dourado, & Carvalho, ; Degidi, Nardi, & Piattelli, ; Henriksson & Jemt, ; Lops, Romeo, Chiapasco, Procopio, & Oteri, ; Lops, Mosca, Müller, Rossi, Rozza, & Romeo, ; Malchiodi, Cucchi, Ghensi, & Nocini, ), while three papers described data collected from cross‐sectional clinical studies (Chang & Wennström, ; Nisapakultorn, Suphanantachat, Silkosessak, & Rattanamongkolgul, ; Palmer, Farkondeh, Palmer, & Wilson, ). Ten studies were conducted in a university (Borges et al., ; Chang & Wennström, ; Choquet et al., ; Cosyn et al., ; Henriksson & Jemt, ; Lops et al., , ; Malchiodi et al., ; Nisapakultorn et al., ; Perez et al., ) one in private practice (Degidi et al., ) and one in both of the settings (Palmer et al., ) (Table ).…”

“…Two studies excluded smoking patients (Degidi et al., ; Lops et al., ), while other two studies excluded smokers >10 (Lops et al., ) and >20 (Malchiodi et al., ) cigarettes per day. Only one study (Choquet et al., ) included two smokers (>20 cig./day), whereas the remaining seven studies did not report on patients’ smoking habit (Borges et al., ; Chang & Wennström, ; Cosyn et al., ; Henriksson & Jemt, ; Nisapakultorn et al., ; Palmer et al., ; Perez et al., ).…”

“…The liability of excess cement remainder in the periodontal area and its consequences has been discussed as an adverse outcome, in case with abutment margin depths and immediate cemented restoration (12), which may be prevented with abutment that allow screw-retained crowns (6). Another disadvantage associated with definitive abutment after surgery is the difficulty of selecting the appropriate definitive standard abutment immediately after implant insertion, concerned to high soft tissue and wall bone variance (12) that can be prevented by using customized abutments (13).…”

“…However, the customized abutments seem to provide better results than angled abutments for ideal crown contours and peri-implant soft tissue support [22]. Among the variety of custom abutments that can be satisfactorily utilized for implant-retained prostheses, the UCLA castable type is one of the most popular.…”
