Background: Substance use is known to be episodic, dynamic, complex, and highly influenced by the environment, therefore a situational and momentary focus to alcohol craving research is appropriate. Current advances in mobile and wearable technology provide novel opportunities for craving research. However, the lack of consensus within craving theory impedes the identification and prioritization of parameters to be monitored. The aim of this study is to critically review current craving models in order to determine viable theoretical frameworks of alcohol craving and its essential parameters. Methods: Eighteen models of craving were reviewed by applying a literature search with a five-step strategy that accounted for the momentary nature of craving and included a snowballing search and a key term extraction algorithm. Based on this review, multiple decision criteria were defined upon which to evaluate the models. Results: Six models for alcohol craving were supported by sufficient empirical research to be eligible. The inferences drawn on these six models resulted in three decision criteria: the model should (1) incorporate negative affect as a predictor of relapse; (2) explain that dependent drinkers have a higher attentional bias towards alcohol cues than nondependent drinkers; (3) incorporate increased risk of relapse with heightened stress levels. Conclusions: The affective processing model of negative reinforcement, the cognitive processing model, the incentive sensitization theory of addiction and the theory of neural opponent motivation are classified as viable theoretical frameworks, resulting in negative affect and stress as relevant parameters to include in real-time craving monitoring research. Anxiety and substance use problems are common and often comorbid, and past research has shown that young adults in particular are especially at risk for developing these disorders. To further delineate the relationship between anxiety and substance use, the current study evaluated anxiety sensitivity (AS)-a cognitive vulnerability factor-as a moderator of the association between trait anxiety and illicit substance use in a large sample of young adults (N = 845; M = 18.7 years, SD = 1.0). It was hypothesized that AS would moderate the association between trait anxiety and illicit substance use, such that trait anxiety would significantly predict illicit substance use among those with high, but not low, AS. Consistent with prediction, a significant trait Anxiety × AS interaction was found, χ²(5) = 29.38, p < .001. Specifically, analyses of simple slopes revealed that for the high-AS group, as trait anxiety increased, so did frequency of illicit substance use (odds ratio [OR] = 1.03, p = .005; 95% confidence interval [CI] [1.01, 1.06]). The slope for the low AS was not significant (OR = 0.98, p = .100; 95% CI [0.95, 1.01]). Results also showed significant moderation effects for the AS physical concerns and cognitive concerns facets but not AS social concerns. These findings suggest that AS may be an important cognitive vulnerability that may help to identify those at particular risk for substance use, and that interventions should target AS reduction in anxiety-prone individuals to reduce and prevent substance abuse.

Relation between physiology and cravingThe use of physiological parameters to determine the state of the mind is not new, where Cacioppo and Tassinary already established first principles of psychophysiology in 1990. These principles can still be considered as authoritative. They cite the first studies that were performed around the end of 1800. In psychophysiology, the nervous system is regarded as having a physiological exit connected to other elements with multiple functional outputs, including verbal, behavioral and contextual data. Cacioppo et al. (2016) explain that there are multiple ways to view the relation between physiological and psychological events (see Figure 1). Psychophysiological relations can be as straightforward and simple as one-on-one EMA design and assessment of the intensity, burden and data quantityCurrently, little research is available to make informed decisions about EMA designs and wearable use, taking into account the intensity, burden, data quantity and data quality in daily life studies (Eisele et al., 2020). This is crucial to the study into the relations between physiology and craving, since a possible limitation of measuring in the "wild" is the incompleteness of both wearable and EMA data. Because the completeness of the data relies on the compliance of respondents and the functioning of the technology, missing values are likely to occur (Shiffman, 2009). If the wearable biosensor is not perceived as usable and comfortable, this could be a reason to stop wearing the sensor. Furthermore, because people diagnosed with AUD are known to value anonymity (Hufford & Shiffman, 2002), wearing a biosensor for an alcohol craving study might make them feel stigmatized. Additionally, a possible drawback of using a repeated intensive assessment like EMA is that the high burden might discourage participation.
