The International Headache Society (IHS) Headache Classification as Applied to a Headache Clinic Population

Abstract: Four hundred patients attending a headache clinic were classified using the IHS criteria. The majority required more than two, often three or four, diagnoses. Even though migraine was the most common diagnosis, only 1/4 of those with a migraine diagnosis had it as the only diagnosis. Seventy-five percent of migraine patients had coexistent chronic tension-type headache (CTTH), drug-induced headache or both. Ninety-six percent of patients diagnosed as having migraine with aura also suffered from migraine withou… Show more

“…It was proposed that more children and adolescents could be classified if a fifth category of comorbid headache was added to the Silberstein criteria. This comorbid headache is similar to the adult phenomenon that the Parma group [10,19,21,22] labeled migraine with interparoxysmal headache. Several authors have proposed that these interparoxysmal headaches are a stage in the evolution of episodic migraine to CDH [11,19,20,23,24].…”

“…In a different population of preschool children, Sillanpaa et al [5] found a prevalence of less than 1%. In adults, the prevalence of daily or near-daily headaches has been estimated to be 4% of the general population [6] and 27% to 39% of specialized headache clinic patients [7][8][9][10].…”

“…The overlap of epidemiological and pathophysiological findings, clinical features and response to treatment suggests that migraine and tension-type headache may in fact represent different manifestations of a pathophysiological continuum, rather than distinct entities [30][31][32]. The present finding of significantly higher prevalences of both types of recurrent headache in TGA patients adds to the evidence for a pathophysiological relationship.…”

“…One third of these patients could not be classified as CTTH because they had too many migrainous features. In a less selected sample, Sanin et al [5] attempted to validate IHS criteria in a clinic population. They randomly selected the clinical records of 400 patients and classified them using the IHS criteria.…”
