The intersection between asthma and acute chest syndrome in children with sickle-cell anaemia

Abstract: Acute chest syndrome is a frequent cause of acute lung disease in children with sickle-cell disease. Asthma is common in children with sickle-cell disease and is associated with increased incidence of vaso-occlusive pain events, acute chest syndrome episodes, and earlier death. Risk factors for asthma exacerbation and an acute chest syndrome episode are similar, and both can present with shortness of breath, chest pain, cough, and wheezing. Despite overlapping risk factors and symptoms, an acute exacerbation o… Show more

“…73,74,76 There is good evidence that asthma is exacerbated by air pollutants, particularly ozone, 77 and there is a strong association between asthma and acute complications of sickle cell disease. 78 The analysis and interpretation of climatic and air-quality effects are complicated by the close correlations among the various factors and the lack of consistency in methodologic and statistical approaches. Furthermore, all studies so far have looked at associations at the population level.…”

“…Our definition of asthma may have underestimated the number of children who actually had asthma because our definition required both a physician diagnosis of asthma and a prescription of asthma medication. A more liberal diagnosis of asthma, commonly used in large epidemiological studies, is a physician only diagnosis of asthma (von Mutius et al , ; Subramanian & Kennedy, ; DeBaun & Strunk, ).…”

“…Since inflammation has an important role in the pathogenesis of vaso-occlusion and asthma augments the pro-inflammatory state, a vicious cycle of inflammation, asthma and vaso-occlusion develops in SCA (27). Asthma among children and adults with SCA is associated with increased incidence of SCA-related morbidity and mortality (27, 28). Studies show elevated levels of leukotrienes in plasma in SCA individuals, which correlates with increased vaso-occlusion and acute chest syndrome (ACS) (29, 30).…”
