The Intra-Pericardial Paraganglioma Presenting as Ascites and Hemopericardium with Impending Tamponade

Abstract: The intra-pericardial paraganglioma is very rare and most of them present with hypertension or palpitations. Here we reported an extraordinarily rare case of intra-pericardial paraganglioma presenting as faint, pitting edema, abdominal fullness with ascites, and hemopericardium with impending tamponade, which was treated successfully by emergent pericardiocentesis and surgical resection.

“…The presence of multiple coronary artery fistulae, which are often observed in patients with cardiac paraganglioma, was noted in our patient. 6,7) Coronary artery fistulae are usually formed because of high-flow blood vessels supplying the tumor, which commonly appear highly concentrated and dilated. 7) However, neovascularization in pheochromocytomas and paragangliomas may be caused by increased production of dopamine and vascular endothelial growth factor.…”

“…Moreover, these tumors infiltrate cardiac tissues or the pericardial space, which can result in features of cardiac tamponade and make resection challenging [5][6][7]. In instances when the atrioventricular groove or extracardiac structures are involved, resection may become impossible [5].…”
