El levantamiento y transporte manual de cargas se asocia a los desórdenes músculo esqueléticos, los cuales representan un alto costo para los sistemas de salud y las empresas. Por tanto se requieren estrategias que mitiguen o eliminen los riesgos derivados por su ejecución. El objetivo de este trabajo fue revisar conceptos actualizados sobre los factores de riesgo, métodos de evaluación, medidas de control y prevención que se asocian al levantamiento y transporte manual de cargas. Metodología: Se revisaron artículos publicados de 2007 - 2013 en las bases de datos EBSCO, PROQUEST Y SCIENCE sobre factores de riesgo, métodos de evaluación, medidas de control y prevención asociadas al levantamiento y transporte manual de cargas. Resultados: Se revisaron 43 artículos, donde se encontró que los factores de riesgos asociados fueron la magnitud de la carga, la altura de la superficie, la frecuencia y el ritmo del levantamiento. Las metodologías de evaluación más usadas fueron el método NIOSH y los análisis de videos a través de software. Las medidas de control y prevención estuvieron relacionadas en su mayoría con el control en los individuos, de las cuales el dispositivo asistencial de levantamiento personal (PLAD) fue el más citado para controlar los riesgos. Conclusiones: El levantamiento de carga constituye un importante factor de riesgo biomecánico en la empresa. Entre las medidas de control y prevención de lesiones en la manipulación manual de cargas, tienen relevancia los controles en el trabajador.

IntroductionThe impact of work-related musculoskeletal disorders is considerable. The assessment of work tasks with physical workloads is crucial to estimate the work-related health risks of exposed employees. Three key indicator methods are available for risk assessment regarding manual lifting, holding and carrying of loads; manual pulling and pushing of loads; and manual handling operations. Three further KIMs for risk assessment regarding whole-body forces, awkward body postures and body movement have been developed de novo. In addition, the development of a newly drafted combined method for mixed exposures is planned. All methods will be validated regarding face validity, reliability, convergent validity, criterion validity and further aspects of utility under practical conditions.Methods and analysisAs part of the joint project MEGAPHYS (multilevel risk assessment of physical workloads), a mixed-methods study is being designed for the validation of KIMs and conducted in companies of different sizes and branches in Germany. Workplaces are documented and analysed by observations, applying KIMs, interviews and assessment of environmental conditions. Furthermore, a survey among the employees at the respective workplaces takes place with standardised questionnaires, interviews and physical examinations. It is intended to include 1200 employees at 120 different workplaces. For analysis of the quality criteria, recommendations of the COSMIN checklist (COnsensus-based Standards for the selection of health Measurement INstruments) will be taken into account.Ethics and disseminationThe study was planned and conducted in accordance with the German Medical Professional Code and the Declaration of Helsinki as well as the German Federal Data Protection Act. The design of the study was approved by ethics committees. We intend to publish the validated KIMs in 2018. Results will be published in peer-reviewed journals, presented at international meetings and disseminated to actual users for practical application.

Sex differences in muscle activity and motor variability in response to a non-fatiguing repetitive screwing task

Background: Musculoskeletal disorders are more prevalent among women than among men, which may be explained by aspects of motor control, including neuromuscular requirements and motor variability. Using an exploratory approach, this study aimed to evaluate sex differences in neuromuscular responses and motor variability during a repetitive task performed on 3 days.Methods: Thirty women and 27 men performed the non-fatiguing, repetitive, 1-h screwing task. For neuromuscular responses, the mean and difference values of static, median, and peak percentile muscle activity levels (normalized to a reference voluntary contraction force) and, for motor variability, the mean and difference values of relative and absolute cycle-to-cycle variability across days were compared between both sexes for each muscle. A mixed-design analysis of variance was used to assess differences between both sexes.Results: The non-fatiguing character of the screwing task was confirmed by the absence of decreased force levels in maximal voluntary contractions performed before and after the task and by absence of electromyographic signs of muscle fatigue. The static and median muscle activity levels tended to be higher among women (on average 7.86 and 27.23 %RVE) than men (on average 6.04 and 26.66 %RVE). Relative motor variability of the flexor and biceps muscles and absolute motor variability of both upper arm muscles were lower in women (on average 0.79 and 29.70 %RVE) than in men (on average 0.89 and 37.55 %RVE). The median activity level of both upper arms muscles tended to decrease within days among women (on average -2.63 %RVE) but increase among men (on average + 1.19 %RVE). Absolute motor variability decreased within days among women (on average -5.32 to -0.34%RVE), whereas it tended to decrease less or increase within days among men (on average -1.21 to + 0.25 %RVE).Conclusion: Women showed higher levels of muscle activity and lower initial relative and absolute motor variability than males when performing the same occupational task, implying women may have a higher risk for developing disorders and point to both sexes using different intrinsic motor control strategies in task performance. Clearly, biological aspects alone cannot explain why women would be at higher risk for developing disorders than men. Therefore, a wider range of individual and environmental factors should be taken into account for optimizing work station designs and organizations by taking into account sex differences.
