The lymph node yield in the initial lateral neck dissection predicts recurrence in the lateral neck of papillary thyroid carcinoma: a revision surgery cohort study

Abstract: Background: This study aimed to evaluate the relationship between lateral lymph node yield (LLNY) and the ratio of lateral positive lymph nodes to lymph node yield (LPLR) from initial lateral neck dissection (LND) in patients with papillary thyroid carcinoma (PTC), as well as the risk of recurrence in patients undergoing LND reoperations. Methods: This retrospective cohort study enrolled patients with PTC who underwent revision LND between 1 January 201… Show more

“…Furthermore, the CCLN on the left side was resected en bloc after the RLN was properly exposed and preserved. In the current study, the median number of retrieved lymph nodes in bilateral CND was 10.5, which is consistent with our previous study suggesting the optimal number (11) in an open bilateral CND [ 15 , 16 ]. Since the indication was very restricted (early stage), our preliminary result showed a satisfactory number of LNY in CND (no less than other endoscopic or open approaches), thus reflecting the thoroughness of this technique [ 8 , 12 ].…”

“…Due to the different extent of dissection, absolute number of lymph nodes with metastasis may not reflect the true situation. Studies have demonstrated the significance of metastatic lymph node ratio (MLNR) in PTC prognosis(44)(45)(46) but it is rarely used in predicting CCLNM up to now. Our study showed that RICLNM was a stronger predictor of CCLNM than the existence of ICLNM.…”
