The Malignancy Potential of Porokeratosis: A Single-Center Retrospective Study

Abstract: BackgroundPorokeratosis (PK) is a rare group of keratinization disorders. While the overall prognosis of PK is favorable, malignant transformation of PK to skin cancer has been reported in 6.9% to 11.6% of the cases. Prior estimates of malignant transformation of PK have been based on reviews of published cases, which introduces possible publication bias. We aim to eliminate this potential bias and quantify the characteristics, risk factors, and malignancy potential of PK. MethodologyA single-center retrospect… Show more

“…Porokeratosis can be a premalignant disease. Malignant transformation occurs in 6.9% to 11.6% of patients [ 14 ]. Squamous cell carcinoma in situ and squamous cell carcinoma are the most common tumors; however, basal cell carcinoma, melanoma, and other cancers have been observed to develop in the porokeratosis lesions.…”

“…Squamous cell carcinoma in situ and squamous cell carcinoma are the most common tumors; however, basal cell carcinoma, melanoma, and other cancers have been observed to develop in the porokeratosis lesions. Therefore, a regularly scheduled follow-up of the reported patient and her DSAP lesions is planned [ 14 ].…”

“…The lesions predominantly arise in White women during the third and fourth decades of life and can greatly alter the quality of life . The DSAP lesions are typically benign; however, malignant transformation is not uncommon and may occur at a higher rate than in the general population and other familial porokeratoses …”

“… 1 Although it is usually classified as a keratinization disorder, this disease could be reclassified as a genodermatosis, since recent studies have revealed that its underlying cause is a heterozygous germline mutation, usually of the genes that encode the enzymes of the mevalonate pathway. 2 Case reports, case series and review articles since the 1970's have associated porokeratosis with malignancy, specifically cutaneous squamous cell carcinomas (cSSCs), 3 but, to date, substantial epidemiological studies on porokeratosis and the risk of skin cancer have not been available.…”
